About this file

This document is an Addendum to a Request for Grant Application (RFGA) issued by the Oregon Department of Human Services, Office of Self-Sufficiency Programs, Survivor Investment Partnership. The purpose of the Addendum is to add a new section for Round 2 Interviews. The Competitive Range has been set at 85 points and higher, and all applications scoring 85 or higher will be contacted to schedule a Round 2 Interview. The interview questions will focus on the applicant's board support, funding history, organizational structure, community outreach, and program success metrics. Applicants will also be required to provide a budget PowerPoint presentation detailing how the Survivor Investment Partnership funds, projected to be $566,848 for the 2023-25 biennium, will be used to provide accessible services and meaningful financial support to historically underserved domestic violence and sexual assault survivors. The application will be evaluated based on a 200-point scoring system across various criteria, including funding history, organizational structure, outreach, and success measures.

a. Is your Board of Directors aware of and in support of this grant request?

b. Funding:

If your organization is currently receiving or has in the past received Federal and/or State funding for Domestic Violence and Sexual Assault (DVSA) survivors, please identify how your organization has used these funds. What were the outcomes?

What were some successes and challenges?

c. Structure:

Please tell us about the structure of your organization. If you have staff that are mandatory reporters, how do you ensure confidentiality and privilege for DVSA survivors? If you do not currently provide DVSA services, do you have Certified Advocates or a plan to ensure employees working with DVSA survivors meet the minimum training requirements set out in OAR 137-085-0080 (Link to OAR:

d. Outreach:

Please tell us about your current community outreach providing culturally responsive services to DVSA survivors in communities that have historically been underserved. e. Success:

How will you measure the success of your program if awarded Survivor Investment Partnership funding, and what specific metrics or outcomes will you use to demonstrate its impact?

3.5.2.1.2 Budget PowerPoint Presentation:

The funds being disbursed through this grant are designated for the 2023-25 biennium and must be spent by June 30, 2025. The Agency may extend the funding through December 31, 2025. SIP funding is projected to be ongoing, but it is contingent on budgetary approval each biennium. If awarded these funds, your organization will receive $566,848 immediately upon the execution of a Grant Agreement with Agency, with the potential to receive additional 2025-27 biennium funds as soon as July 2025. Please tell us how these funds will align with SIP’s intention to provide accessible services and meaningful financial support to DVSA survivors within populations and communities that have historically been underserved. Please explain how you will ensure this funding is spent within this timeframe and walk us through your proposed spend-down plan. What are potential challenges you foresee in implementing these funds into your program, and how do you plan to address them?
