About this file

This is a Business Reference Form attachment for a Massachusetts state contract opportunity to provide culturally and linguistically appropriate support for readability and usability. The form outlines the documentation requirements that bidders must submit as corporate references to demonstrate their qualifications and relevant experience. Bidders are required to provide comprehensive information for each corporate reference, including the client organization's legal name and address, parent company or doing-business-as designations if applicable, and client size metrics such as number of customers, employees, and revenue figures where available. Additionally, bidders must supply detailed contact information for a primary point of contact at the reference client, including name, title, address, telephone number, and email address.

The reference form requires bidders to document the scope and scale of their previous work, specifically requesting a description of contract requirements and an enumeration of products and services delivered. Bidders must also provide project timeline information, including contract start and end dates, and staffing details encompassing both the project team size and the ongoing operations team size. For applicable contracts, bidders must report the number of members or clients served on a monthly basis during operations and the annual cost associated with ongoing operations. This comprehensive documentation enables the contracting agency to verify bidders' experience, organizational capacity, and successful delivery of similar services.
