About this file

The document is a Request for Proposal (RFP) 25-020 issued by the Salem Area Mass Transit District (SAMTD) for Project Management Software. The RFP includes six exhibits (Exhibits #1-6) that bidders must complete and submit, including a Bid Form, Non-Collusion Affidavit, Organizational Conflict of Interest form, Contractor's Representations and Warranties Certification, and a Federal Debarment & Suspension Certificate. The proposal requires bidders to provide detailed information about their company, including business classifications, tax identification numbers, and references demonstrating successful contract performance similar in size and scope to the project.

The exhibits require bidders to disclose potential conflicts of interest, certify compliance with state and federal tax laws, and verify that they are not debarred or suspended from federal contracts. Bidders must submit typed, legible documents with digital signatures preferred, and failure to include any required information or modify the exhibits will result in the bid being deemed non-responsive. The RFP emphasizes transparency, legal compliance, and thorough documentation from potential contractors, with specific requirements for verifying the bidder's financial standing, operational history, and ability to perform the proposed project management software contract.
