About this file
This is an RFP Submission Checklist for the City of Philadelphia, Department of Aviation, designed to assist applicants in submitting complete and responsive proposals for Professional Services opportunities at or above the formal RFP threshold. The checklist provides a comprehensive guide for applicants to verify compliance with all submission requirements prior to deadline, including confirmation that proposals adequately address the Scope of Work requirements, incorporate a compliant Cost Proposal, and meet all format and content standards outlined in the RFP Section III. Applicants must review all RFP documents and Contract Attachments, including the General Provisions, and disclose any objections within their proposals, understanding that objections are not favored and may negatively impact evaluation.
The checklist requires applicants to submit Appendix C (City of Philadelphia Tax and Regulatory Status and Clearance Statement), a Statement of Financial Capacity along with supporting documentation such as audited or unaudited financial statements, bankruptcy disclosures covering the past five years, and IRS Form 990 for non-profit organizations. All submissions must be completed electronically through eContractPhilly with the correct opportunity number, properly signed by an authorized signatory, and the applicant's registered name and Tax Identification Number must match across the applicant profile, Appendix C, and W-9 form. Optional submissions include Appendix D (Local Business Entity or Local Impact Certification) and Appendix E (LGBTQ Applicant Opportunity Data), and all applicants must disclose information required under Philadelphia Code Chapter 17-1400 regarding local political contributions through their eContract Philly accounts.
