About this file

This document is a Commercial Reference Past Performance Questionnaire for RFP W519TC-25-R-0034, designed to evaluate a contractor's performance across multiple key assessment areas. The questionnaire provides a comprehensive rating system with six primary evaluation categories: Quality, Schedule, Cost Control, Management, Utilization of Small Business, and Regulatory Compliance, using a performance rating scale from Exceptional to Unsatisfactory.

The questionnaire is related to a Draft Request for Proposal for a Small Caliber Second Source ammunition production contract, seeking a supplier to produce 5.56mm, 7.62mm, and Caliber .50 ammunition. The procurement is a restricted competition limited to the United States, its outlying areas, or Canada, with a specific requirement that the supplier be geographically separated from the Lake City Army Ammunition Plant. The Draft RFP is being issued to obtain industry feedback, with comments due by July 18, 2025, and the Final RFP scheduled for release in August 2025. The solicitation is managed by the Department of the Army Materiel Command Contracting Command at Rock Island Arsenal.
