Therefore, it is important that your information be as factual, accurate and complete as possible to preclude the need for follow-up by the evaluators. Please rate the risk associated with each survey item by checking the appropriate box. Use the following rating scale:
Very Low Risk (VL): Essentially no doubt exists, based on the Offeror's performance and systemic improvement record, that the Offeror will successfully perform the required effort.
Low Risk (L): Little doubt exists, based on the Offeror's performance and systemic improvement record, that the Offeror will successfully perform the required effort.
Moderate Risk (M): Some doubt exists, based on the Offeror's performance and systemic improvement record, that the Offeror will successfully perform the required effort.
High Risk (H): Significant doubt exists, based on the Offeror's performance and systemic improvement record, that the Offeror will successfully perform the required effort.
Unknown Risk (U): Little or no relevant performance record identifiable; equates to an unknown risk rating having no positive or negative evaluation significance.
