About this file

This document is a Past Performance Questionnaire (PPQ) for a Veterans Health Administration (VHA) contract opportunity related to renovating Building 73 at the Minneapolis VA campus. The PPQ is designed to evaluate a contractor's performance across seven key metrics: quality, schedule/timeliness, communication, management/personnel, cost/financial management, subcontract management, and overall performance. The form requires detailed information about previous contract work, including contract details, project scope, performance dates, contract pricing, and allows the previous client/evaluator to provide adjectival ratings ranging from Outstanding to Unsatisfactory for each performance category. The questionnaire is intended to be completed by a previous client and submitted as part of the proposal for the Building 73 interior renovation project, which involves design-build services for replacing finishes, repairing HVAC components, and constructing interior partitions to improve space efficiency.
