About this file
This document is a Request for Proposal (RFP No. 250000001038) issued by Michigan.gov/MiProcurement for a Digital Microfilm Scanner Full-Service Preventative Maintenance Agreement. The RFP involves maintenance services for microfilm scanning equipment used by the Michigan Department of Health and Human Services (MDHHS) Vital Records Control, which currently has seven existing units - four within their 3-year warranty period and three outside of warranty. The proposal seeks comprehensive maintenance and on-site repair services for these digital microfilm scanning units.
The pricing schedule requires contractors to submit detailed pricing for three specific line items: annual maintenance during the 3-year warranty period, annual maintenance after the warranty period, and on-site repair services. The State of Michigan reserves the right to modify contract items to meet its needs and those of MiDeal members, other states, and political subdivisions. Preference will be given to bidders who can provide both scanning equipment and full-service preventative maintenance. Contractors must submit pricing that includes all potential costs, and they must certify that their pricing was determined independently without communication with other bidders.

4. By submitting its proposal, the Contractor certifies that the prices were arrived at independently, and without consultation, communication, or agreement with any other Contractor.
