About this file

This document is an Exhibit B for RFP 2026_001, focusing on company background and software description requirements for a potential Enterprise Resource Planning (ERP) system solicitation by the CRC (Cooperative Purchasing Consortium). The document provides a comprehensive template for vendors to submit detailed information about their company, software solution, and organizational capabilities, with instructions for completing a proposal section that will be used to evaluate vendor stability, technical capabilities, and fit for the project.

The RFP seeks a cloud-based or alternative system delivery method for an ERP solution, with vendors required to provide extensive details including company history, employee breakdown, client sector composition, financial stability, research and development investment, and potential legal/contractual history. Key evaluation criteria include the vendor's ability to support contract commitments, software module descriptions, licensing options, system integration capabilities, and infrastructure details. The document emphasizes that incomplete or non-responsive submissions may be rejected, and vendors must transparently disclose any past contract terminations, legal actions, workforce reductions, or financial challenges from the previous five years.

COMPANY BACKGROUND & SOFTWARE DESCRIPTION

Instructions

Use the word document beginning on the next page to enter your responses, along with your written narrative of the requesting information. The completed form and narrative should become Section 2 of your proposal.

Vendors must provide information about the company so that the CRC can evaluate the Vendor's stability and ability to support the commitments set forth in response to the RFP. The CRC, at its option, may require the Vendor to provide additional documentation to support and/or clarify requested information.

Complete the basic information in the tables and provide a narrative that outlines the company's background including a brief description (e.g., past history, present status, future plans, company size, etc.). Describe the software offered as a solution, including any hardware requirements. This is the vendor’s opportunity to tell the company’s story, what their mission is, what they are passionate about and how they best fit the needs of the CRC.

Indicate if at any time during the past five years Vendor has had a contract terminated for convenience, non-performance, or any other reason, or has entered into legal action with a customer. Describe the situation(s) including name and address of contracting party.

If a submitted RFP includes blank responses or the narrative is not submitted, the document may be considered non-responsive and rejected.

The remainder of this page is intentionally left blank.

The CRC would prefer a cloud-based system, but will consider all system delivery methods, including hosted systems or managed services. If the system is cloud based, describe the system SaaS infrastructure and where it is located, preferably within the United States.

If the proposer’s solution requires new or additional hardware or equipment, the CRC intends to purchase this separately from this proposal.
