About this file
This document contains required submission forms and insurance guidelines for the City of Roswell RFQ 24-305-C Fire Protection System Services. Bidders must submit multiple affidavits and certifications to be considered for award, including compliance with Georgia's Security and Immigration Compliance Act (O.C.G.A. 13-10-91), which requires verification of federal E-Verify registration and authorization throughout the contract period. Additional mandatory submissions include a non-collusion affidavit, oath and affirmation regarding the accuracy of bid information, certification of ability to meet all RFP terms, and drug-free workplace compliance documentation. The document specifies that proposals may be withdrawn in writing prior to the due date, and the City reserves the right to negotiate contract modifications with the apparent winning offeror within one week of notification to resolve any contractual differences.
Professional services insurance requirements apply to projects under $5,000,000 and include statutory workers' compensation with specified liability limits, commercial general liability of $1,000,000 per occurrence, auto liability of $500,000 per occurrence, and professional liability of $1,000,000 per claim/aggregate. The City of Roswell must be named as an additional insured on general liability and auto liability policies, and all insurance carriers except workers' compensation must maintain an A.M. Best Rating of A-5 or higher and be licensed in Georgia. Certificates of Insurance must reference the specific project name and bid number, provide 30 days cancellation notice, and contractors must waive subrogation rights against the City. Contractors are required to maintain comprehensive equipment and contents insurance on a replacement cost basis and comply with OSHA regulations and industry-accepted risk management practices.
