QUALITY ASSURANCE SURVEILLANCE PLAN

FCDAS Boilers Replacement Construction

Fairbanks Command and Data Acquisition Station (FCDAS)

PURPOSE

This Quality Assurance Surveillance Plan (QASP) provides a standard of surveillance for monitoring the requirements of the above listed contract and a systematic approach for conducting surveillance over the performance aspects of the effort. The QASP is for the use of Government Quality Assurance (QA) personnel to insure standards of the contract are being met.

The QASP provides a systematic method to evaluate the services the Contractor is required to furnish, but not the details of how the Contractor accomplishes the work. This plan uses inspections and customer feedback as the primary evaluation criteria. This surveillance assures the Government that the Contractor’s performance is acceptable.

Any noncompliance with contract performance requirements is deemed ―sub-par. The term ―sub-par refers to a support service that does not meet the standard of performance specified in the contract. The QASP is based upon the premise that the Contractor, not the Government, is responsible for management and quality control actions to meet the terms and conditions of the contract. The methods of surveillance recognize that unforeseen and uncontrollable problems do occur however the Contractor shall employ adequate resources to identify and correct these problems. Effective management and use of a quality control plan will allow the Contractor to operate within the specified surveillance requirements.

Government’s quality assurance methods are expected to be objective, fair, and consistent in evaluating Contractor performance against the standards.

PROCEDURE

Quality Assurance Schedule The QA will develop a checklist with evaluation procedures to use when conducting surveillance of the Contractor’s performance as identified in the QASP.

Surveillance

The QA will assess Contractor’s completion of the tasks outlined in the Performance Work Statement (PWS) from review of product submittals and datasheets and verifying that the equipment and materials installed are in accordance with contract drawings and specifications. Functionality and proper operation of the installed equipment, servers, workstations, sensors, wiring, system notifications and alarms etc.

shall be verified through testing and physical verification. The QA will develop an opinion of the task completion and the quality of the work completed based on thorough review of submittals, testing reports, pictures, monitoring and control of sensors, controllers and equipment utilizing Building Management System (BMS) through software and Human Machine Interface (HMI). The QA will determine whether or not the performance meets the standards contained in the contract.

First, the submittals, test reports, shop drawings and installed equipment, sensors, wiring and controllers shall be compared to contract specifications and drawings. Then the review will focus on completeness of work, in individual buildings, as well as the quality of the completed work. A log will be developed to document review of all reports and submittals. Government comments on the submittals shall be provided to the Contractor asking him to revise reports and other submittals to address provided comments or provide clarification to the Government regarding the comments.

The QA is required to accept or declare as sub-par Contractor services and determine amount of payment due to the Contractor.

METHODS OF SURVEILLANCE

Assessments

Periodic assessments will occur on a pre-determined basis to interact with the Contractor at critical stages of the project such as after field surveys and submission of product submittals as well as installation and integration of Building Management System (BMS) components into the new BMS system. The results of periodic assessments will be used to covey to the Contractor that their performance is sub-par and to improve quality of produced deliverables.

The QA is the point of contact for the QASP and shall collect all customer feedback and comments. All comments and resultant resolutions shall be documented and become a permanent part of the QA surveillance records. Valid, documented negative feedback may be used as the basis for asking the Contractor to improve their performance and positive feedback may be used to encourage the Contractor to continue performing above expectations.
