o Each box or container should be opened and compared to the packing slip to ensure that you received exactly what is listed on the packing slip.

o You should inspect and count the internal items of each box or container for concealed damage, quantity, and serial number accuracy.

o If you find a discrepancy on any of these, a ROD (Report of Discrepancy) is required.

o You should complete an ROD for each discrepancy found per shipment.

o The ROD form can be found on the CoMSupCen website (Located under the Download Center, Forms Folder).

o Once you have completed the ROD Form, it should be emailed or faxed back to your respective DSO representative at the Consequence Management Support Center.

o DSO will log the ROD and assign a ROD identification number so that it can be tracked in our quality program. Each ROD will be investigated and corrected.

o Each piece of equipment that is received should also be inspected and tested for functionality.

o You should complete a ROD Form for any piece of equipment that does not function properly.

o Once you have completed the ROD Form, it should be emailed or faxed back to your respective DSO representative at the Consequence Management Support Center.

o DSO will log the ROD and assign a ROD identification number so that it can be tracked in our quality program. Each ROD will be investigated and corrected.

o You should also use the Support Center Reach back Support numbers to troubleshoot the equipment. It may be an issue that can be resolved over the phone instead of having to exchange the equipment.

o All ROD entries will be logged and tracked in order for us to improve our process, which will in turn improve the efficiency of each team. The following procedure is for the exporting of items via PDF or Excel.

o From the “Current Orders/Status” screen, select “Export to PDF/Excel” at the top of the screen.

o You will then be prompted to Open/Save/Cancel.

o Select “Save” and choose a destination folder that fits your needs.

o Once saved, the document can be viewed, printed, and stored for future use, i.e. (SEAT).

Teams will to go to the Help Tab and select “NG Assistance Request Entry” and enter information.

• Typical reasons Teams would need to submit an AR, there is an additional event their Team is tasked with and additional equipment is needed to perform the event, or if there is a need to replace unserviceable equipment that needs to be submitted for repairs.

• Before doing this, once the Team identifies unserviceable equipment, the Team will contact the CoMSupCen’s Technical Repair Shop to troubleshoot. If the Shop is unable to resolve the issue, the Team will submit a repair ticket in the website.

• As Teams enter information as it pertains to their need for a Temp Loan of equipment, remind them to entry as much information as possible to better allow NGB and CoMSupCen to understand their needs.

o Who: Team(s) and or personnel involved.

o What: What equipment is needed specifically and or any special requirements.

o When: When is it needed no later than (NLT), for how log and expected return date if know.

o Where: Where is the event and shipped to address?

o Why: Identify the event or requirement that justifies the need to borrow equipment. Or if there is a 2407 created based upon trouble shooting with the CoMSupCen’s Technical Repair Shop.

o How: How will this Temp Loan be managed?

• Teams will be temporary loaned equipment 30 to 90 days depending on the event dates or the Estimated Delivery Date of the 2407 of their equipment repair.

• DSO Reps will monitor the Due Back Date of the assigned Task. If Temp Loan is waiting on 2407 to be repaired, DSO Rep will monitor the EDD and follow up with the CoMSupCen’s Technical Repair Shop or APO to validate EDD. Notes will be entered on 2407 to identify current status of repairs at a minimum of every two weeks. If the EDD will exceed the Due Back Date, a request for extension will be emailed to the DSO Manager who will request an extension from NGB.

• DSO Reps will begin coordinating the return of Temp Loan at a minimum of two weeks prior to Due Back Date. If the 2407 EDD is completed and returned prior to the due back date, DSO Rep will begin coordinating return of equipment once Team verifies received and functionality of repaired equipment. DSO Rep will add notes to Task during this process to show actions taken to include any tracking numbers for returns.

• Once Temp Loan is returned, it there are any discrepancies such as shortages, Receiving will submit a Report of Discrepancy (ROD) and DSO Rep will be responsible for recovering shortages or resolving discrepancies. Team will be given 30 days for provide shortages or resolve discrepancies. After that period, Team will submit A0A/A0E through the web to order shortages and have shipped to CoMSupCen. Supply and Service Requisitions

Requisitions received:

• After approvals are received by Commander, NGB, Contracts (if applicable), and COR (if applicable), the requirements are annotated on DA form 2765/1348 (or a CoMSupCen Materiel Release Order (COMMRO) for internal purchases).

• Requisitions are assigned on a daily basis to a Purchasing agent by the Contractor based on commodity (and/or workload) for which each person is responsible.
