About this file
This is a Proposal Requirements document for the City of St. Petersburg, Florida's request for fully insured group insurance coverage including Basic Group Term Life/Accidental Death and Dismemberment (AD&D), Voluntary Supplemental Life, Spouse and Child Voluntary Supplemental Life, Voluntary Personal AD&D, and Long Term Disability (LTD) plans. The City seeks proposals from qualified insurance carriers on a sole source carrier basis, with offerors required to quote all Plan options presented and identify any differences between proposed plans and the City's current coverage. Proposals must include detailed Plan designs with sample enrollment packages (minimally a Plan brochure), manuals, and informational materials demonstrating the firm's expertise. The document specifies comprehensive requirements for each coverage type, including Basic Life with automatic enrollment for newly eligible employees without medical underwriting, retiree coverage provisions with specified age reductions, and employer-paid coverage for active employees.
The City's evaluation will prioritize the most competitive offer based on benefits and pricing to achieve the lowest long-term net cost to both the City and its eligible employees, retirees, and dependents. Current coverage is provided through Standard Insurance. Key plan specifications include 100% employer-paid Basic Life and LTD coverage; guaranteed issue supplemental life insurance during initial eligibility periods and qualified Family Status Changes with medical underwriting for subsequent enrollment; voluntary employee-paid supplemental and AD&D coverage with specified benefit levels by employee classification and salary ranges; Long Term Disability with an "Own Occupation" definition for the first 24 months, a 180-day elimination period, and a benefit period extending to age 65, SSNRA, or 5 years whichever is longest; and coordinated short-term disability benefits with the City's Extended Illness Leave program. The City requires offerors to handle all phases and costs of medical underwriting when necessary and provide withholding and remittance of all related taxes on disability payments.

Plan Designs & Differences
