About this file

This Base Agreement is a contract between the City of St. Petersburg, Florida, and a qualified insurance carrier to provide ancillary insurance services including basic group term life/accidental death and dismemberment (AD&D), voluntary supplemental life coverage (for employees, spouses, and children), voluntary personal AD&D, and long-term disability (LTD) plans for eligible city employees, retirees, and their dependents. The initial contract term commences upon execution and continues for three years, with the City reserving the right to renew the agreement for two additional one-year periods under the same terms and conditions, contingent upon mutual written agreement by both parties. The contractor must be a licensed insurance carrier (not agent) per Florida Department of Insurance regulations with a minimum of five years of experience providing group life and AD&D administration in Florida, and must demonstrate the ability to obtain the minimum insurance requirements outlined in the agreement while being free from legal and regulatory matters that would prevent fulfillment of contractual obligations.

The contractor shall invoice the City monthly for services rendered in accordance with rates specified in Appendix C, with payment due within thirty days of invoice receipt; the total compensation, inclusive of all out-of-pocket expenses, shall not exceed a specified maximum annual price. Rates are guaranteed as firm for the initial three-year term, and any rate adjustment requests must be submitted in writing at least six months in advance of a new plan year, subject to written City approval. The contractor must maintain comprehensive insurance coverage including commercial general liability of at least $1,000,000 per occurrence and $2,000,000 aggregate, commercial automobile liability of $1,000,000 combined single limit, workers' compensation as required by Florida law, and errors and omissions/professional liability insurance of at least $1,000,000 per occurrence. The City makes no enrollment guarantees and requires that the contractor provide rates guaranteed regardless of enrollment numbers or enrollment mix, while providing guarantee-issued coverage to all currently covered members and new hires for basic and supplemental life coverage during the new employee enrollment period.
