About this file

This is a Prevailing Wage Payroll Certification Form issued by the Connecticut Department of Labor, Wage and Workplace Standards Division, applicable to public works projects within Connecticut. The form serves as the official document for contractors and subcontractors to certify weekly payroll compliance with Connecticut General Statutes Section 31-53, which mandates prevailing wage requirements for all employees on public works projects. Contractors must submit certified payrolls monthly to the contracting agency and are required to provide a copy of the OSHA 10 Hour Construction Safety and Health Card for each employee, attached to the first certified payroll submission. The form captures detailed weekly payroll information including worker names, classifications, hourly rates, daily hours worked, gross pay, fringe benefits, deductions, and net pay for all personnel performing work on the project.

The form requires contractors to certify under penalty of felony that all wages paid meet or exceed the prevailing wage rates established by the Labor Commissioner, that all required fringe benefits and contributions to employee welfare funds have been paid, that each employee maintains active worker's compensation insurance coverage, and that no kickbacks have been received. Fringe benefits covered under the certification include medical or hospital care, pension or retirement plans, life insurance, disability coverage, vacation and holiday pay, and other specified benefits. Contractors must also ensure compliance with all provisions of Connecticut General Statutes Section 31-53 and Section 31-54 if applicable to state highway construction projects. Falsification of certified payroll documents constitutes a Class D felony punishable by fines up to five thousand dollars and imprisonment up to five years. The form is designated a public document and explicitly prohibits the inclusion of Social Security numbers.
