About this file

This is a Prevailing Wage Payroll Certification Form issued by the Connecticut Department of Labor, Wage and Workplace Standards Division, for use on public works projects in Connecticut. The form is submitted monthly by contractors and subcontractors to the contracting agency and applies to the Town of Manchester's Traffic Signal Maintenance project (Bid No. 26/27-13), with bids due November 6, 2026. The form requires detailed weekly payroll information including employee names, classifications, hourly rates, hours worked each day, gross pay, deductions, and fringe benefits for all workers on the project. Contractors must submit a copy of the OSHA 10 Hour Construction Safety and Health Card for each employee attached to the first certified payroll submission.

The form mandates compliance with Connecticut General Statutes Section 31-53 regarding prevailing wage requirements, worker's compensation insurance coverage, and fringe benefits. All wages paid must meet or exceed the prevailing wage rates determined by the Labor Commissioner, with fringe benefits including medical care, pension, life insurance, disability, vacation, and holidays as applicable. The employer must certify under penalty of law that all submitted records are accurate, no kickbacks have been received, and all employees are properly covered by workers' compensation insurance. The certification statement carries significant legal consequences, as filing false payroll information constitutes a Class D felony subject to fines up to five thousand dollars and imprisonment up to five years. The form is a public document and must not include employee Social Security numbers.
