About this file

This is a Past Performance Reference Information Form template that requires offerors to document their relevant contract experience when responding to solicitations.

The form requires offerors to provide comprehensive details about prior contracts including the offeror's name and UEI, the reference client name and UEI, the contract number and title, and the contract period of performance. Offerors must identify the contract type (such as Fixed-Price, Time and Materials, Cost Reimbursement, IDIQ, BPA, or BOA) and provide the total contract value with approximate average annual value and the value performed to date as of the RFP release date. A primary point of contact from the reference contract—specifically the Contract/Subcontract Lead or equivalent—must be identified with their title, telephone, address, and email. The complexity section requires offerors to summarize the scope, work difficulty, similarity to the current solicitation, and any problems encountered with creative solutions. Additionally, offerors must demonstrate commitment to safe work practices by providing relevant safety program information, safety performance indicators such as Experience Modification Rates for the past three years where applicable, and disclosure of regulatory citations from OSHA, Cal-OSHA, or equivalent international authorities within the past three years if readily available. The completed form is limited to three pages per reference contract, and offerors may amend the format provided they maintain exact information, font, and size requirements. The form does not mandate provision of all safety information, though the evaluating organization reserves the right to request clarification on the offeror's ability to perform work safely and responsibly.

Where applicable, the Offeror may also provide general information regarding the safety practices of any proposed lower-tier subcontractors.

To support an understanding of the Offeror’s safety approach, the Offeror is encouraged to provide available safety performance indicators, such as Experience Modification Rates (EMR) for the most recent three years, where applicable to the business unit(s) performing the Work. If provided, any EMR exceeding 1.0 should be accompanied by a brief explanation.

Disclosure of regulatory citations (including OSHA, Cal-OSHA, or equivalent international safety authorities) within the past three years is requested only to the extent such information is readily available and deemed relevant by the Offeror. Where disclosed, a high-level summary of the nature of the citation and its resolution status is sufficient. Detailed reporting is not required.
