About this file
This document is a Past Performance Questionnaire for GSA Request for Proposal (RFP) 47PF0025R0019 for Masonry Restoration and Cornice Repointing. The questionnaire is designed to collect detailed performance feedback from a previous project reference, with a submission deadline of April 16, 2025, to be sent directly to the GSA Contracting Officer, Rhonda Booker.
The questionnaire comprehensively evaluates the contractor's performance across multiple dimensions, including quality, budget, schedule, management and staffing, administration, and overall performance. It requires the reference to provide ratings on a 5-point scale and optional narrative explanations for various aspects such as contract conformance, budget management, schedule adherence, subcontractor management, communication effectiveness, client sensitivity, and administrative responsiveness. The form is structured to capture nuanced insights into the contractor's past performance, which will likely be used in evaluating future contract awards.
