About this file

This is a Past Performance Questionnaire template designed to collect performance evaluation data from government assessors regarding contractors' or subcontractors' prior performance on relevant contracts. The questionnaire pertains to solicitation 1305M326Q0399 issued by NOAA's Acquisitions and Grants Office for the Woods Hole, MA Facility Backup Generator and Transfer Switch Recommissioning project.

The form is structured in four sections: Section I captures solicitation and project data; Section II requires contractors to provide current or historical contract information (including contract number, type, value, period of performance, and project description) for assessor validation; Section III collects assessor identification and role information; and Section IV requests an overall performance rating using a five-tier scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) based on criteria including contract requirement fulfillment, quality and timeliness of delivery, cost management, communications, and customer satisfaction. Offerors must complete Sections I and II, submit copies with their proposals by the solicitation closing date, and provide the questionnaire to government assessors who previously worked with them on relevant contracts. Completed questionnaires should be returned to jenna.taulman@noaa.gov. This is a template document requiring completion rather than a document specifying deliverables or requirements.
