About this file

This is a utility outage coordination form used by the 802d Civil Engineer Squadron at Fort Sam Houston (FSH), Randolph Air Force Base (RND), and Lackland Air Force Base (LAK) to manage planned and unplanned utility outages.

The form requires contractors to document outage details including the date, permit number, work/project number, primary utility type (electrical, HVAC, fire alarm, fire suppression, water, sewer, gas, or other), secondary affected systems, affected facilities, outage duration, proposed date with alternates, start and end times, work description, specific facility room/floor/area affected, requestor contact information, and 802 CES Project Manager contact information. The form mandates pre-coordination with external contractors (American Water for water/sewer, CPS Energy for electrical/gas, SAWS, 502d Communications Squadron, Network Enterprise Command, Army Security System Network for FSH and Cannon Barracks only, Security Forces Squadron, and 902 CES Fire Emergency Service) and internal facility managers for all affected buildings. Additionally, coordination is required with multiple CES supervisory positions including Interior/Exterior Electric, Alarm, Power Production, Plumbing, Water/Wastewater, EMCS, and HVAC supervisors, along with the CE Customer Service and Service Contracts supervisors, Facilities Chief, Infrastructure Chief, and final approval from the 502 CES Chief and 802 CES Director/Deputy. The process requires a 30-day notification period after approval, with all pre-coordinators signing their respective pages before submission to the 802 CES Project Manager, who then routes it through 502 CES to 802 CES Leadership for final approval.
