About this file
This is an Invitation for Bid (IFB) form submitted as an offer to the State of Illinois for Genetic Testing services under solicitation number 26-478HFS-CHSUP-B-49843. The document serves as the bidder's formal submission of their proposal to perform services in full compliance with the subject solicitation requirements. The form requires bidders to confirm completion of all required documents as specified in solicitation document section A.1, with the understanding that failure to meet all requirements will result in disqualification. The document references Standard Terms and Conditions that will become part of any resulting contract, which are available on the Illinois Procurement website at https://cpo-general.illinois.gov/solicitation-and-contract-templates.html.
The form addresses preferences available under the Illinois Procurement Code and provides bidders the opportunity to claim applicable preferences such as the Illinois Business Preference or Domestic Product Preference, with corresponding documentation and line item designations required. Bidders may take exceptions to standard contract terms and conditions by submitting a redlined version of the Standard Terms and Conditions document with their offer, subject to State acceptance or rejection. The document is incomplete in the provided file, with multiple signature fields and data fields remaining unfilled, indicating this is a blank template form rather than a completed bid submission.
