About this file
This is an Invitation for Bid offer submission form for the State of Illinois, specifically for the Department of Central Management Services (CMS). The solicitation pertains to the establishment of a five-year contract for uninterrupted power supply (UPS) inspection, repair, and maintenance services across multiple locations throughout Illinois. The bid number is 26-416CMS-BOPM4-B-51027. The document functions as a standard state bidding form requiring vendors to submit their offers in compliance with all solicitation requirements, with failure to meet specified requirements resulting in automatic disqualification.
The form addresses bidder qualifications, preferences available under the Illinois Procurement Code, and exceptions to standard contract terms and conditions. Bidders claiming Illinois Business Preference must complete and submit the required affidavit, while those claiming Domestic Product Preference must designate "Domestic Product" in the alternate description field for applicable line items. All resulting contracts will incorporate Standard Terms and Conditions available through the Illinois Procurement Office website. The document requires authorized representatives to certify their bid submission and compliance with solicitation requirements, with the State of Illinois reserving the right to accept or reject any proposed exceptions to standard contractual language.
