About this file

This is a Traffic Control/Roadway Work Permit form issued by the New Mexico Department of Transportation (NMDOT) for managing traffic control operations during roadway construction and maintenance activities. The permit requires contractors to provide comprehensive project information including the contractor name, traffic control firm details, certified traffic control supervisor contact information, work zone location (route, mile post, intersections, and direction), roadway configuration (2-lane through 8-lane, divided or undivided), existing and proposed speed limits, and working duration with specific start and end dates and daily work hours. The permit identifies the purpose of work through categories such as roadway construction/rehabilitation, shoulder work, signal and lighting work, utility work, drainage/excavation, soil testing, signing and striping, or other activities. A Traffic Control Plan (TCP) must be enclosed with the permit submission for processing approval. The NMDOT District Office requires contractors to confirm actual start dates prior to commencing work, and the permit becomes effective only upon approval by the designated District Traffic Section.

The permit establishes binding conditions upon contractor submission, including compliance with the Manual on Uniform Traffic Control Devices (MUTCD) standards, contractor indemnification of NMDOT and its employees against liability and claims arising from negligent acts, and provision of a certified insurance policy naming NMDOT as an additional-named insured with primary coverage. The NMDOT District Office reviews permits and issues approval with or without amendments, assigns a permit number, and coordinates with the District Public Information Officer. Contractors and TCP firms must strictly adhere to all attached notes and conditions referenced in the permit approval. The form provides space for documenting approval status, conditions, and signatures from NMDOT representatives along with submission dates to the Public Information Officer.
