SECTION SF 30 BLOCK 14 CONTINUATION PAGE
SUMMARY OF CHANGES
SECTION L - INSTRUCTIONS, CONDITIONS AND NOTICES TO BIDDERS
The following have been modified:
SECTION L
INSTRUCTIONS TO OFFERORS
PART A: GENERAL INSTRUCTIONS
1.0 GENERAL
The Offeror must respond to all requirements of the solicitation and not alter or rearrange the solicitation. The Offeror is advised that the Government may incorporate any portions of the Offeror’s proposal into the resulting contract.
In presenting material in the proposal, the Offeror is advised that quality of information is more important than quantity. Clarity, brevity, and logical organization shall be emphasized during proposal preparation. Statements that the prospective Offeror understands, can comply with, or will comply with, the specifications, and paraphrasing the requirements or parts thereof without supporting information are considered inadequate by the Government, and may result in assessing weaknesses and/or deficiencies.
The Offeror must include any data that illustrates the adequacy of the various assumptions, approaches, and solutions to problems. Failure to address a specific factor or subfactor clearly may be considered a deficiency. Unnecessarily elaborate brochures or other presentation materials beyond that sufficient to present a complete and effective proposal is neither necessary nor desired.
The Government advises the Offeror that taking exception to or deviating from any term or condition of the RFP may result in the assessment of a deficiency to the proposal.
Throughout these instructions, “Offeror” is defined as the prime contractor with its CAGE code identified in Block 15A on Standard Form (SF) 33, Solicitation offer and Award.
2.0 ELECTRONIC PROPOSAL FORMAT
This section is intended to provide information to the Offerors on the electronic format and application software to be used for submitting proposals. Do not submit hardcopy proposals for this solicitation. Use of the software and procedures described in this section will reduce the amount of time and effort needed to receive and upload proposals and will ensure the proposals received are suitable for reading electronically during evaluation.
Proposals must be formatted using a Times New Roman 12 pt Normal font (no reduction permitted), single-spaced with 1-inch margins all around, and formatted for standard 8.5 x 11 inch paper. All pages should be numbered with section and page numbers. Graphs and tables shall be presented in no smaller than a 10 pt font and should contain a grid, which allows values to be read directly from the graph. Graphs and Tables that fit on an 8.5” X 14” printed page will be counted as one page. Drawing may be of any size and will count as one page. Graphic resolution, including such data as tables or charts, should be consistent with the purpose of the data presented. When using Government supplied templates (e.g. Attachment P3, Attachment P6, Attachment P7), no modifications to font or scaling required. All proposal documents must be compatible with Microsoft Office 2016 and/or the latest Adobe Acrobat Reader Portable Document Format (PDF), where applicable.
The Offeror is responsible for ensuring electronic proposals are virus free and shall run an anti-virus scan before submission. Offerors may use hyperlinks within and among proposal volumes that do not include price information. However, there shall be no links from any other volume into the Price Volume or into any other portion of the proposal that contains price information. In order to reduce file sizes, the Offeror shall not embed sound or video (e.g., MPEG) files within the proposal submission. The Offeror is encouraged to simplify the color palette used in creating figures; and minimize size of graphics files; and avoid scanned images.
All electronic files submitted shall follow the following naming convention:
For Volume 1 Corporate Experience proposal electronic files: Vol_1_XXX For Volume 2 Technical proposal electronic files: Vol_2_XXX For Volume 3 Past Performance proposal electronic files: Vol_3_XXX For Volume 4 Price proposal electronic files: Vol_4_XXX
The XXX represents the appropriate title the Offeror can identify. Naming files in the format above will allow the Government to store electronic files for each volume accordingly.
3.0 PROPOSAL CONTENT AND VOLUMES
The Offeror must present proposal information in a manner that facilitates a one-to-one comparison between the information presented and this Proposal Instruction. Proposal information must be structured such that its Volume/paragraph number matches the Proposal Instructions Volume/paragraph number provided in section “Part B Specific Instructions” to which it is responding, although the Offeror may add lower tier subparagraphs. The Offeror must provide reasons it will not provide information for a particular paragraph. The proposal information instructions are structured by paragraph numbers where first, second, third, and fourth parts correspond to the volume, section, and element, etc., in the Offeror’s proposal.
Each volume of the proposal shall be submitted as specified in the table below. The table below supersedes the copies requested in Block 9 of the Standard Form (SF) 33. All volumes of the proposal shall be received electronically via the Solicitation Module in PIEE prior to the closing date/time stated in this solicitation. 4.0 ELECTRONIC PROPOSAL SUBMISSION:
Electronic copies of each volume shall be submitted through the Solicitation Module of PIEE at https://piee.eb.mil. No other submissions, such as mail, hand-carried, proprietary or DOD SAFE submissions, or proposals submitted by mail or hand-carried will be evaluated. If there is a system outage, see below. All proposals received after the exact time specified for receipt shall be treated as late submissions. For instructions on how to post an offer, please refer to the Posting Offer demo: https://pieetraining.eb.mil/wbt/sol/Posting_Offer.pdf.
It is the Offeror’s responsibility to obtain written confirmation of receipt of all electronic files of the full proposal by the Contracting office. In the event that the Solicitation Module is down, the alternate method for proposal submission is identified in Paragraph 4.1 below.
The submission date for all Volumes shall be no later than the date and time specified in Block 9 of the SF 33 of the RFP.
4.1 In the unlikely event the PIEE system is not operational, experiences technical difficulties, or a Contractor is temporarily unable to access or use the system, the Contractor shall immediately notify the Contracting Officer. This Notification must occur prior to the proposal submission deadline. Contractor notification shall be in writing and may be in conjunction with verbal notification, but verbal notification alone shall not be sufficient.
In the unlikely event the PIEE system is not operational, the alternate method for proposal submission is via expedited delivery or hand-carried if the Contractor has current base access. The Offeror must obtain prior approval from the Contracting Officer to use the alternate submission method. The Contractor shall have 24 hours to provide their proposal upon contracting officer approval.
In the event that the Contractor alleges technical difficulties and does not notify the contracting officer until after the closing date and time, the contracting officer will follow the procedures identified in the Federal Acquisition Regulation (FAR) and then make a determination if the Contractor’s late proposal submission is accepted. Screen shots of the submission should be taken to validate a submission was accepted in the PIEE system.
5.0 RESERVED
6.0 CLASSIFIED DATA
All proposals must be UNCLASSIFIED.
7.0 SOLICITATION CHANGES
For notice of any changes and additional information provided by the Government for the solicitation, please go to https://sam.gov/content/home.
8.0 SOLICITATION QUESTIONS/COMMENTS
Questions regarding the solicitation should be submitted in PIEE via the Contracting Communication Module (CCM) by utilizing the Solicitation Questions and Answer Form, Attachment L-1. Each question or comment should reference the applicable document, page number, and paragraph number. Offerors shall not include any proprietary information in a question, because the Government will provide the question and answer to all potential Offerors. All questions must be submitted no later than 4:00 PM Eastern Time on the 14th calendar day after the date that this solicitation is issued. The Government is under no obligation to answer questions received after that date, but may answer questions if time permits.
PART B SPECIFIC INSTRUCTIONS
Cost or pricing information shall only appear in the Price volume.
1.0 VOLUME 1: CORPORATE EXPERIENCE (PASS/FAIL CRITERIA)
1. The Offeror shall complete Attachment L-2 according to the instructions within the attachment. Only applicable contract references should be used in completing the attachment.
1.1 CORPORATE EXPERIENCE
1. The Offeror shall identify and submit no more than two (2) contract reference(s) that meet the criteria in a) below and collectively meets the criteria of b) and c) below. For instance, the Offeror may submit only one (1) contract reference if all criteria is satisfied within one (1) contract reference. Alternatively, the Offeror may submit one (1) contract reference that satisfies criteria a) and b), and one (1) contract reference that satisfies criteria a) and c).
a) The Offeror’s corporate experience reference demonstrates performance as the Prime or principal Sub-Contractor and the contract meets the definition of Recency as defined in Section M, OR the Offeror's principal subcontractor corporate experience reference demonstrates performance as the Prime and meets the definition of Recency. A “principal subcontractor” for purposes of corporate experience is defined as a subcontract experience totaling at least 20% of the total contract price; AND
b) The Offeror’s contract reference demonstrates development and delivery of a training analysis product, as defined in SOW 3.5.1 and 3.5.3, for a military aviation community; AND
c) The Offeror’s contract reference demonstrates development and delivery of a training evaluation product, as defined in SOW 3.5.2 and 3.5.3, for a military aviation community.
2. The Offeror shall submit documentation clearly illustrating the Offeror has a minimum of a Secret facility security clearance. The Offeror may satisfy this requirement by providing documentation from the Prime or from the Joint Venture member performing the classified scope of work. This requirement may NOT be satisfied by a principal subcontractor.
3. The Offeror shall not submit information on individual employee experience of the entities as part of this proposal; the Government will not consider this information in the Corporate Experience evaluation.
4. This procurement does not have key personnel requirements and the Government will not consider experience of individual employee(s) as a part of corporate experience
1.2 CORPORATE REFERENCE SUBSTANTIATION
1. For the contract reference included in Attachment L-2, the Offeror shall provide the following in the Volume 1 Annex A:
a) A complete copy of the contractually executed final version of the substantiating documents (drafts are not acceptable) including the latest conformed contract, AND statement of work (SOW) or performance work statement (PWS) that demonstrate compliance with the Corporate Experience criteria.
b) A requirement not specifically identified in the SOW/PWS or otherwise supported through documents as discussed herein will not be considered to have demonstrated compliance with this Corporate Experience criterion.
2. Within Annex A, the contracts referenced in Attachment L-2 should be clearly identified and the Offeror shall highlight, in yellow, the portions of the documents that clearly identify that the contract reference meets the criteria in Section L, Part B, paragraph 1.1. The Offeror shall denote which highlighted areas correspond to each requirement within Attachment L-2. The Government may consider non-highlighted portions of the supporting documentation or contracts, but the Government has no duty to search for missing pieces of supporting documentation or to search parts of the supporting documentation that is not highlighted and cited by the Offeror to cure the problems it finds in the information provided by the Offeror.
1.3 Additional Instructions for Certain Small Business Concerns.
In accordance with sections 15(e)(5) and 8(d)(17) of the Small Business Act, the Government may consider certain experience of small business offerors that have been first tier subcontractors and/or members of joint ventures.
Joint Venture experience: A prime offeror that has been a member of a joint venture may elect to use the experience of the joint venture (whether or not the other joint venture partners were small business concerns) where the small business does not independently demonstrate experience necessary for award. An offeror submitting experience under this provision shall:
(A) Identify the joint venture of which the small business concern is or was a member;
(B) Identify the contract(s) of the joint venture that the offeror is electing to use for its experience; and
(C) Inform the contracting officer what duties and responsibilities the offeror carried out or is carrying out as part of the joint venture. To be considered, the work must have been performed by the offeror and not another member of the joint venture.
First Tier Subcontractor Experience: if a small business offeror performed as a first-tier subcontractor on a contract that included a subcontracting plan and elects to use that experience in response to this solicitation, the contracting officer shall consider the concern's experience as a part of the evaluation. The Offeror must provide the documentation required above and a copy of the subcontracting plan.
2.0 VOLUME 2: TECHNICAL (Factor 1)
Note: This volume shall not contain any reference to price aspects of the offer.
1. Sample task scenarios are provided for which the Offeror shall propose according to the instructions listed in paragraphs 2.1 and 2.2.
2.1 MASTER TASK LIST APPROACH
2.1.1 Technical Approach. Based upon the GPSMAP 64 Series / GPSMAP 64X Series Owner’s Manual (attachment L-3), the Offeror shall create a Master Task List (MTL). The Offeror shall submit the MTL in Microsoft Excel format. The MTL, at a minimum, shall include: a) duties, b) tasks, c) subtasks, d) requisite knowledge, skills, and attitude (KSA) data.
2.2 TRAINING TASK LIST SELECTION APPROACH
2.2.1 Technical Approach. Based upon the provided scenario data in attachment L-4, the Offeror shall outline its approach for identifying which tasks require training and should be included in the Training Task List. The technical approach shall include methods to determine tasks requiring a) “overtraining”, b) “training”, and c) “no formal training”. The Offeror shall also complete attachment L-4 and identify which tasks should be selected for training, overtraining, and no formal training based upon the Offeror’s technical approach and provided task scenario data.
3.0 VOLUME 3: PAST PERFORMANCE (Factor 2)
3.1 PAST PERFORMANCE OVERVIEW
a) Offeror Summary Table: The Offeror shall complete the table below. The Offeror shall include all subcontractors and JV team members who will be involved with the performance of the contract. If a JV teaming or subcontracting arrangement is proposed, identify the work share, distribution elements, and ratios that each contractor will perform using the table below. Also provide a definition of the legal relationship between the entities if it is other than a Prime/subcontractor relationship.
Contractor Name (Indicate Prime, JV Team Member, or Sub)
CAGE CODE/UEI
Brief Work Description and/or Program Responsibility
% of Total Proposed Price
e.g. ABC Inc., Sub $2.9M
VWXYZ/12-3456789
SOW 3.5.1, 3.5.3
20%
XYZ, Prime $11.6M
ABCDE/98-7654321
SOW 3.5.2, 3.5.3
80%
The table above shall include the dollar value proposed under the Contractor’s name. Adding up the dollar values identified in this table shall equal the total proposed price. The percentages identified by the Offeror and subcontractors shall add up to 100%.
Within the “Brief Work Description and/or Program Responsibility” block, Offerors shall also include the applicable paragraph numbers as they relate to the relevant SOW paragraphs that will be reviewed for Relevancy in Past Performance: Training Analyses (SOW 3.5.1), Training System Evaluation (SOW 3.5.2), and Products/Documents/Reports (3.5.3). If a subcontractor’s proposed role does not fit within one of these SOW paragraphs, please state that fact.
3.2 PAST PERFORMANCE SUBMISSION INFORMATION
(a) Sources of past performance evaluation information include information provided by the Offeror in response to the solicitation, information obtained from questionnaires, and any other sources available to the Government, to include, but not limited to, the Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information Systems, Electronic Subcontract Reporting System, or other databases; the Defense Contract Management Agency; and interviews with Program Managers, Contracting Officers, and Fee Determining Officials. While the Government may have access to these sources, the Government has no duty to search for missing pieces of supporting documentation (as defined in Section L.B.3.3a ) to cure the problems it finds in the information provided by the Offeror. The Offeror has the burden of providing thorough and complete past performance information.
(b) The Offeror shall complete the Past Performance Contract Reference Worksheet, Attachment L-8, for each Past Performance Contract Reference (i.e. contract or delivery/task order) performed within five (5) years of the solicitation release date. The Offeror shall include no more than three (3) Past Performance Contract References (Government or commercial) for the Government’s evaluation consisting of the Offeror’s, JV members and principal subcontractor’(s) relevant past performance. Of the three (3) total Past Performance Contract References, the Offeror shall include a maximum of two (2) references from principal subcontractors. As part of the Past Performance Contract References, the Offeror shall resubmit the same contract record(s) that were used to satisfy the corporate experience requirements. A “principal subcontractor” for purposes of the past performance evaluation factor is defined as a subcontract experience totaling at least 20% of the total contract price.
(c) The Offeror shall NOT submit Past Performance Contract References that include classified supporting documentation, as the Government needs to be able to easily obtain and review information pertaining to classified past performance.
(d) The Government is evaluating past performance of the Offeror (i.e., entity identified in Block 15A of Standard Form 33) and any principal subcontractor proposed to perform at least 20% of the instant solicitation requirements and total contract price. An Offeror will not receive past performance credit for a proposed principal subcontractor unless the Offeror’s proposal demonstrates how the resources of that principal subcontractor – its workforce, management, facilities or other resources – will in fact be provided to perform at least 20% of the total contract price. Merely stating that the Offeror has access to the resources of a principal subcontractor is insufficient. The Government will not evaluate the past performance of any company other than the prime, JV members and principal subcontractor(s). Past Performance Contract References where supplies and services were sold or transferred between corporate divisions, subsidiaries, or affiliates of a contractor will not be considered unless the interorganizational transfer is identified in the form of a written agreement between the corporate divisions, subsidiaries, or affiliates -- such as a subcontract, purchase agreement, modification incorporating a novation agreement, job order, or similar type written agreement – with the prime, JV members’ or principal subcontractor's CAGE code, UEI number, or corporate name and address on the cover page of the written agreement. Absent a written agreement, a signed statement from a corporate officer of the entity that holds the Past Performance Contract Reference must be provided outlining the work that was performed by the separate corporate division, subsidiary, or affiliate. Past Performance Contract References that the contractor performed as part of an interorganizational transfer will be evaluated in the same manner as if the contractor performed in a subcontractor role. In such instances, the written agreement or signed statement from a corporate officer between the corporate divisions, subsidiaries, or affiliates must specify the exact requirements the contractor is responsible for executing as part of the prime contract (utilizing prime contract supporting documentation as defined in this solicitation) in order to evaluate the scope and magnitude of the effort performed.
(e) Any submitted Past Performance Contract Reference must contain performance within five (5) years of the RFP release date specified in Block 5 of Standard Form 33 of the RFP. Performance that took place greater than five (5) years from the RFP issuance date will not be considered unless a portion of the performance falls within the five (5) years stated herein. The entire contract period of performance will be considered, even if only a portion of performance falls within the five (5) year period, with the understanding that more recent performance is generally more relevant than less recent performance. Submitted Past Performance Contract References without performance within this specified period will not be considered relevant by the Government.
(f) It is possible that an older Past Performance Contract Reference with performance that barely crosses into the five-year period (see above) could be determined Not Relevant, depending upon the work that was actually delivered or provided during that time period. For example, for a trainer delivery solicitation, a past performance contract that performed trainer deliveries could be determined Not Relevant if the only thing being provided or delivered during the period of time that crosses the five-year mark is maintenance services.
(g) The Offeror should provide written consent from its principal subcontractors and/or individual JV members that will allow the Government to coordinate any of those entities’ past performance issues with the Offeror. If the Offeror does not submit such written consent, then the Government will address any past performance issues directly with the principal subcontractor and/or JV members and the Offeror will forfeit the opportunity to participate in any related discussions. Consequently, for any principal subcontractors and/or JV members that do not provide such written consent, provide a point of contact name, address, phone number and email address with whom the Government may coordinate past performance issues.
(h) The Offeror has the burden of providing thorough and complete past performance contract information as set forth in the Past Performance Contract Reference Worksheet (Atch L-8). It is incumbent upon the Offeror to explain the relevance of data provided in its proposal, however, statements that cannot be clearly validated within an Offeror’s provided supporting documentation will not be given credit towards an Offeror’s past performance relevancy rating assignment. The Government has no duty to search for additional data to cure problems the Government finds in the information provided by the Offeror. For Past Performance Contract References that are still ongoing in performance, the Offeror shall demonstrate that relevant delivery has been made on those contracts.
(i) TOs/DOs awarded under Multiple Award Contracts (MACs) will be considered standalone “contracts” for purposes of past performance evaluation. Thus, Offerors may not propose an entire MAC as a Past Performance Contract Reference.
(j) TOs/DOs awarded under a Single Award IDIQ Contract will be considered standalone “contracts” for purposes of past performance evaluation. Thus, Offerors may not propose an entire Single Award IDIQ as a Past Performance Contract Reference.
3.3 RELEVANCY INFORMATION OF THE PAST PERFORMANCE CONTRACT REFERENCES
For each Past Performance Contract Reference, provide the following information on the “Atch (L-6) Relevancy Information of the PP Contract”.
(a) Supporting documentation. To support the prime, JV members’ and principal subcontractor’s comparison of the Past Performance Contract References’ product or service similarities to the instant solicitation’s work, Offerors shall provide one copy of the fully executed contract, subcontract, or interorganizational transfer written agreement and its Statement of Work, Performance Work Statement, Specifications, Statement of Objectives, and/or Contract Data Requirements Lists (CDRLs). For subcontract or inter-organizational transfer efforts, the subcontract SOW or equivalent document shall be provided. If the prime or subcontractor performed as a subcontractor on the reference, a copy of the subcontractor agreement between the prime and the subcontractor is required to demonstrate the agreement between the two parties and the division of work. The Government does not want to receive invoices, DD250s, or any other document that is not an attachment to the Past Performance Contract Reference. The copies of the past performance contract reference shall be provided in fully searchable files (MS Word or .pdf) as required in Section L. Offerors are required to cite specific paragraphs and contract CLIN(s) from their provided documentation in the corresponding Attachment L-6 worksheet to support the prime and principal subcontractor’s comparison of the Past Performance Contract References’ similarities to the instant solicitation’s work. Moreover, the Offeror must highlight (in the supporting documentation it provides) the exact content in those cited paragraphs that it believes is similar to the solicitation requirements. If Offerors cite specific references on Military Handbooks or other governing documents that outline courseware development processes, those documents must be referenced in a SOW, PWS, or SPEC to be considered as part of the supporting documentation for use in the evaluation. Documentation such as solicitations, requests for proposals, unsigned contract documentation (e.g. modifications, contract cover pages), partial paragraphs from statements of work or specifications, and redacted Section B CLIN pricing does not provide the Government with adequate information to make a relevancy determination. Do not redact pricing information from supporting documentation.
3.4 DEMONSTRATED PERFORMANCE INFORMATION OF THE PAST PERFORMANCE CONTRACT REFERENCES
a) For each Past Performance Contract Reference, the Offeror shall provide available CPARS, CPAQs, and other past performance information obtained in accordance with 13 CFR 125.11 when applicable, that address the following demonstrated past performance areas:
Quality of Product or Service. The prime or principal subcontractor’s past performance in the delivery of quality supplies and services, which includes meeting technical requirements.
Schedule. The prime or principal subcontractor’s past performance in meeting schedule requirements, to include on-time or late deliveries, modifications of original schedules.
Program Management and Subcontract Management. The prime or principal subcontractor’s past performance in managing the contracted effort, to include Program Management, subcontract management, and cooperation with the customer.
b) The Government will use CPARS, CPAQs and, and other past performance information obtained in accordance with 13 CFR 125.11 when applicable, to document demonstrated past performance. The Government does not want to receive, and will not use for evaluation, an Offeror’s self-described assertions of demonstrated past performance. For the CPARS and CPAQs provided, however, explain the reasons for any past performance problems, including schedule slips, and cost or price increases from time of award. Also describe any claims or requests for equitable adjustment.
c) The Offeror shall forward one (1) copy of Attachment L-5 CPAQ, to the Past Performance Contract Reference customer’s Procuring Contracting Officer (PCO), Administrative Contracting Officer (ACO), and Program Manager who is the Assessing Official ─ via email and carbon copy (cc:) to Mr. Evan Denove, Sr. Contract Specialist, via email at evan.t.denove.civ@us.navy.mil ─ for the following Past Performance Contract References: Past Performance Contract References that do not have CPARS Reports, and Past Performance Contract References where the most recent CPARS Report completed by the Assessing Official is dated more than 12 months prior to the RFP’s release date and is not the final CPARS Report. Furthermore, if the Offeror or principal subcontractor submits a Past Performance Contract Reference where the Offeror or principal subcontractor performed as a subcontractor, the Offeror or principal subcontractor shall instead forward one (1) copy of Attachment L-5 CPAQ, to the Past Performance Contract Reference Prime contractor Program Manager. In addition, if the Offeror or principal subcontractor submits a Past Performance Contract Reference where the Offeror or principal subcontractor performed as a subcontractor, the Offeror or principal subcontract may also forward (1) copy of Attachment L-5 CPAQ, to their Government representative for completion. For clarity, this additional CPAQ to the Government representative does not replace the requirement for the Offeror or principal subcontractor submitting a Past Performance Contract Reference where the Offeror or principal subcontractor performed as a subcontractor to provide Attachment L-5 CPAQ, to the Past Performance Contract Reference Prime contractor Program Manager. A past performance contract’s CPAR completed on a prime contractor will not be considered for the Offeror or principal subcontractor performing as a subcontractor. The Offeror shall make sure that its Government and prime contractor Program Manager, if applicable, Past Performance POCs are current to avoid any delays in the completion and submission of the CPAQs to the Contract Specialist.
d) It is requested that all CPAQs are forwarded within 14 calendar days from the RFP release date, with a copy forwarded to the Contract Specialist. The Offeror shall include instructions to send completed questionnaires within two weeks of the customer’s receipt of the questionnaire to Mr. Evan Denove, Sr. Contract Specialist, via email at evan.t.denove.civ@us.navy.mil, and carbon copy to both chi.t.pham.civ@us.navy.mil and marcus.j.ravaglia.civ@us.navy.mil. The Offeror shall NOT complete the CPAQ, but submit to the point of contacts identified above for completion; and then those individuals are to return it to Mr. Evan Denove directly via email. The Government will not evaluate an Offeror’s submitted CPAQs that are not eligible to be submitted under this instruction.
e) Regardless of relevancy, the Offeror shall provide a list of Show Cause Notices, Cure Notices, and Terminations for Default received on any contract, task order, or delivery order within the past two (2) years of the RFP release date. A brief description of the scope and magnitude of the contract, task order or delivery order shall be provided.
f) For any past performance problem identified in the CPARS or CPAQs (as per b above), describe the status of the problem. If the problem was resolved, describe the resolution used to correct the past performance problem and, if applicable, how the Offeror proposes to use that resolution for this solicitation’s effort.
3.5 Additional Instructions for Past Performance Ratings for Certain Small Business Concerns.
In accordance with sections 15(e)(5) and 8(d)(17) of the Small Business Act, the Government is required to consider past performance of certain small business offerors that have been first tier subcontractors and/or members of joint ventures.
Joint Venture past performance: A prime offeror that has been a member of a joint venture may elect to use the past performance of the joint venture (whether or not the other joint venture partners were small business concerns) where the small business does not independently demonstrate past performance necessary for award. An offeror submitting past performance under this provision shall:
(A) Identify the joint venture of which the small business concern is or was a member;
(B) Identify the contract(s) of the joint venture that the offeror is electing to use for its past performance; and

Any apparent imbalances in pricing, high or low, or any other anomalies, should be fully explained.
(f) Offerors shall propose its estimated prices provided for the CLINs identified in the table below. The prices shown in the table below are only an estimate to show the even split (without escalation) in requirements among the five ordering years. The Offerors shall propose their own pricing.
