About this file

This document is a Visitor Access and Badge Request form for the Puget Sound Naval Shipyard and Intermediate Maintenance Facility (PSNS & IMF) in Bremerton, Washington.

A trusted agent must submit the form at least seven days prior to any visit via Microsoft Teams or encrypted email to designated PSNS & IMF security personnel. The form collects company or command information, visitor personnel details, security clearance levels, and badge type requirements. Four badge categories are available: White Badge (requiring escort and security plan), Green Badge (general unclassified access to Controlled Industrial Areas or detachments), Yellow Badge (access to UNNPI and controlled nuclear information areas, requiring DoD or DOE clearance), and Red Badge (access to nuclear work areas and reactor compartments, requiring DoD or DOE clearance and restricted data training). Visitors and contractors are limited to one-year access periods, while tenant personnel and forces afloat are eligible for three-year access. The sponsor must be a PSNS & IMF employee or approved trusted agent. Non-classified visitors must provide proof of U.S. citizenship. For detachment access at Everett, San Diego, or Japan (Yokosuka), forms must be submitted to designated site security representatives. The form is controlled as CUI (Controlled Unclassified Information) under the Privacy category with limited dissemination control.
