RFP N40083-14-R-2710; MECHANICAL CONSTRUCTION
MULTIPLE AWARD CONTRACT at the NAVAL SUPPORT
ACTIVITY, CRANE IN
AMENDMENT 03: Questions/Clarifications 04/30/2014
PHASE I
QUESTIONS/RESPONSES:
1- Can you comment as to what has transpired since we dropped off our RFP yesterday?
Response: We did not receive enough proposals to continue the RFP for the mechanical construction multiple award contract as a small business set-aside. We had to amend the RFP to an unrestricted procurement.
2-Do I need to sign the acknowledgement and return page #3 from the RFP amendment to you?
Response: The amendment can be acknowledged when you submit the additional requirements.
3- Phase I, Factor 5 Past Performance in Utilization of Small Business Concerns.
Factor 5 references providing performance evaluations, SF1420, DD2626 or equivalent. I am not familiar with those forms. Do I need to get them somewhere and complete them for each job with subcontractors in Factor 3?
Response: The form SF 1420 (this form is no longer applicable), from DD 2626 is a performance evaluation form. Please use Attachment B NAVFAC/USACE PAST PERFORMANCE QUSTIONNAIRE (FORMPPQ-0). This may not be applicable IF you are a small business and self-perform. If you have small business sub-contractors, this would be applicable.
For this Factor 5 items a –g, if an item is not applicable, just state so.
4- If there were projects initially selected with no subcontracted work on those initially prepared evaluations (Exhibit B) included in the RFP I dropped off yesterday, should we select another project with subcontractors?
Response: If you have projects with small business sub-contractors, yes it would be beneficial to provide the information.
5- If we do not have national or industry awards, or MPA agreements, no use of CRP organizations, or if we are not utilizing affiliates, parent, LLC/or LTD member companies, then do we just so state that in our proposal in Factor 5?
Response: Yes, just state.
6-Since we are a small business with no significant prior history in government contracts it seems that most of the factor 5 section may not apply to us?
Response: Yes, correct
7- I see we do need to complete a Small Business past performance Exhibit C, but on what contracts?
Response: Yes, the form states “with no prior history shall complete this form”, complete according (i.e.
$0.00, 0%)
8-Are these Exhibit Cs supposed to tie into our Exhibit B performance evaluations that we selected?
Response: Not necessarily but they may be.
9-It sounds like the proposal we dropped off yesterday will not be evaluated. As such, do we need to pick those RFPs up from you or will they be returned via mail? If I need to pick them up then we will need to coordinate gate passes again.
Response: You can pick the proposals up or just submit additional information that complies with Amendment 0002.
Please address all FACTORS/forms and if the information is not applicable just indicate accordingly.
All questions must be submitted in writing to Annette Taylor @ annette.taylor@navy.mil
