About this file

This document is a Quality Assurance Surveillance Plan (QASP) Matrix for the rental of temporary HVAC and exhaust systems at Pearl Harbor Naval Shipyard (PNSY). The QASP establishes performance standards and acceptable quality levels (AQL) for seven key deliverables and services: technical support, defective equipment repair, equipment start-up and commissioning, preventative maintenance, equipment delivery condition, personnel training, and preventative maintenance planning. All deliverables require 100% compliance with zero deviations from referenced procedures unless approved by PNSY, with surveillance methods including validated customer complaints, user surveys, periodic inspections, and documentation review.

The matrix employs a five-tier descriptive rating system to evaluate contractor performance. An "Exceptional" rating requires flawless execution, proactive communication with well-researched solutions, and early delivery of all components and documentation. "Very Good" performance exceeds some contractual requirements with effective corrective action and clear documentation. "Satisfactory" performance, the expected baseline, meets all contractual requirements with minor non-conformances and timely corrective actions. "Marginal" performance fails to meet some requirements, such as missing key test parameters or repeatedly late documentation, triggering increased government oversight. "Unsatisfactory" performance fails to meet most contractual requirements with multiple significant failures or critical uncorrected deficiencies that jeopardize project completion.

RatingsDescriptive Rating System
ExceptionalPerformance meets all contractual requirements and exceeds many to the Government’s benefit. Problems are rare, and the contractor takes highly effective corrective action. For this SOW, an "Exceptional" rating would require flawless execution, proactive communication of potential issues with well-researched solutions, and delivery of all components and documentation ahead of schedule.
Very GoodPerformance meets all contractual requirements and exceeds some to the Government’s benefit. Problems are infrequent, and the contractor demonstrates effective corrective action. For example, all technical requirements are met, and the contractor provides exceptionally clear and well-organized documentation that facilitates rapid government review.
SatisfactoryPerformance meets all contractual requirements. This is the expected level of performance. Any non-conformances are minor, and the contractor's proposed corrective actions are timely and effective. The work is technically sound, and all deliverables are submitted on time.
MarginalPerformance does not meet some contractual requirements. A "Marginal" rating may be assigned if a significant non-conformance is identified (e.g., failure to meet a key test parameter like breakaway torque) or if required documentation is repeatedly late or incomplete. This reflects a need for increased government oversight.
UnsatisfactoryPerformance does not meet most contractual requirements, and recovery is not likely in a timely manner. This rating would be assigned for multiple significant failures, a critical uncorrected deficiency, or a failure to provide required OQE, which jeopardizes the entire project.
