About this file
This is a Quality Assurance Surveillance Plan (QASP) Matrix for diesel generator preventive and remedial maintenance services at Portsmouth Naval Shipyard. The matrix establishes performance standards, acceptable quality levels (AQLs), surveillance methods, and consequences for non-compliance across multiple service requirements.
Key performance requirements include: (1) Remedial/Emergent Maintenance with telephone response within one hour and on-site response within two hours, available 24/7/365, with no more than one failure per month for telephone response and 100% compliance for on-site response; (2) Operational Restoration during Minor Preventive Maintenance requiring restoration within five minutes with 100% compliance; (3) Preventive Maintenance Timeliness requiring completion within five business days of the scheduled service date with 100% compliance; (4) Authorization for Unscheduled Repairs requiring detailed written cost estimates and prior written approval before work begins; (5) Security Compliance requiring personnel rosters submitted within five business days of contract award with visible identification and compliance with Portsmouth Naval Shipyard security requirements; and (6) Safety and Hazardous Energy Control requiring adherence to OSHA and OSHECM Chapter 250 requirements with accident/incident reporting within one hour. Failure to meet performance standards may result in Contract Discrepancy Reports (CDRs), payment withholding, financial reimbursement requirements, denial of personnel access, work stoppages, and negative performance ratings. The QASP employs a five-tier performance rating system ranging from Exceptional (exceeding requirements with proactive communication) to Unsatisfactory (failing to meet most requirements with unlikely recovery).

Sheet1
| Quality Assurance Surveillance Plan, Matrix | ||||
| Deliverable or Service Requirement | Performance Standards | Acceptable Quality Level (AQL) | Method of Surveillance | Government action for Failure to Meet AQL |
| PWS 3.4 & 3.9.2: Remedial / Emergent Maintenance Response | Telephone Response: Acknowledge service request via telephone within one (1) hour of Government notification.2. On-Site Response (Emergent): Contractor personnel must arrive on-site at the specified generator within two (2) hours of an "Emergent" service call. Available 24/7/365. | Telephone Response: No more than 1 failure per month. On-Site Response: 100% compliance. Failure to meet the 2-hour window for any emergent call is unacceptable due to the critical nature of the equipment. | The COR will keep track of service calls and the time of the contractor's telephonic and on-site response and document within surveillance log/monthly report. | Failure to meet timeframes may result in a Contract Discrepancy Report (CDR). Per SOW 3.9.2, the Government may seek financial reimbursement if it must use its own resources to satisfy the need. |
| Operational Restoration during Minor PM 3.9.5 | During Minor PMs, the contractor must be able to restore operational integrity of the generator within five (5) minutes if required. If this timeframe cannot be met, the contractor must notify the Government immediately before proceeding. | 100% compliance. Failure to meet the 5-minute restoration time without prior notification and Government approval is unacceptable. | Direct Observation: The COR or Designated TPOC will be present during minor PMs on critical equipment and will determine adequate timeliness with documentation if it becomes neccessary. | Failure to complete work on time without approval will result in a CDR and may impact payment. |
| Preventive Maintenance (PM) Timeliness 3.9.1 | Scheduled PM services must be completed within five (5) business days of the scheduled service date, unless a deviation is pre-approved in writing by the Government. | 100% compliance. | Inspection of all submitted test reports and data. | Failure to complete work on time without approval will result in a CDR and may impact payment. |
| Authorization for Unscheduled Repairs (3.5,3.6,3.7) | A detailed written cost estimate on the Work Request Form (Attachment 1) must be submitted for all unscheduled/emergency repairs.2. Work shall not begin until written authorization is received from the KO or designated COR. | 100% compliance. No unscheduled work shall be performed without a prior, written, and approved Work Request Form. | Inspection: COR will review invoices for unscheduled work and verify that a corresponding, approved Work Request Form (attachment 1) is on file before payment acceptance. amy work performed without authorization will be documented. | Per SOW 3.7.2, any work performed without or beyond authorization is done at the contractor's own risk and may not be reimbursed. Invoices for unauthorized work will be rejected. |
| PWS 4.4: Security Compliance | Submit personnel roster within five (5) business days of contract award and for any personnel changes. Ensure personnel wear visible identification and comply with all PNSY security requirements (vehicle, PEDs, etc.). | 100% compliant | Periodic Inspections by COR/TPOC/Qualified Government Representative | Non-compliance can result in denial of access for personnel, causing work delays at the contractor's expense. Will be documented in a CDR. |
| PWS 4.5 & Appendix 3: Safety and Hazardous Energy Control | Adhere to all OSHA and OSHECM Chapter 250 requirements, including use of PPE. 3. Report accidents/incidents within one (1) hour. | 100% compliant | Direct Observation/Periodic Inspection by COR/TPOC/Qualified Government Representative | Any safety violation may result in an immediate work stoppage. Serious violations will result in a CDR and may affect the contractor's performance rating and future contract eligibility. |
| Ratings | Descriptive Rating System |
| Exceptional | Performance meets all contractual requirements and exceeds many to the Government’s benefit. Problems are rare, and the contractor takes highly effective corrective action. For this SOW, an "Exceptional" rating would require flawless execution, proactive communication of potential issues with well-researched solutions, and delivery of all components and documentation ahead of schedule. |
| Very Good | Performance meets all contractual requirements and exceeds some to the Government’s benefit. Problems are infrequent, and the contractor demonstrates effective corrective action. For example, all technical requirements are met, and the contractor provides exceptionally clear and well-organized documentation that facilitates rapid government review. |
| Satisfactory | Performance meets all contractual requirements. This is the expected level of performance. Any non-conformances are minor, and the contractor's proposed corrective actions are timely and effective. The work is technically sound, and all deliverables are submitted on time. |
| Marginal | Performance does not meet some contractual requirements. A "Marginal" rating may be assigned if a significant non-conformance is identified (e.g., failure to meet a key test parameter like breakaway torque) or if required documentation is repeatedly late or incomplete. This reflects a need for increased government oversight. |
| Unsatisfactory | Performance does not meet most contractual requirements, and recovery is not likely in a timely manner. This rating would be assigned for multiple significant failures, a critical uncorrected deficiency, or a failure to provide required OQE, which jeopardizes the entire project. |
