Instructions **NOTE: This tab contains instructions for completing Tables ES-1 and ES-2**
| Instructions for the ES-1 Offeror Summary Table | |
| ***Note to contract specialist | Update the table headers and footers with pertinent solicitation information. From the toolbar, Select View and then Page Layout from the Workbook Views to insert Header and Footer information. |
Row 3 Fill in the Offeror's Name.
Column A List the prime, all subcontractors (principal subcontractors, critical subcontractors, and all other subcontractors), JV team members, and others who will be involved with performance or management of the requirements of this solicitation. Add rows as necessary.
Column B List all Places of Performance, being careful to include accurate physical locations.
***Note to contract specialist Place one of the following notes in the Column B Instructions, depending on the solicitations circumstances:
1. If the work is to be performed at a Government site, include the following: Note: Place of Performance for this solicitation pertains to the physical location of the prime, principal subcontractor, critical subcontractor, and JV team members that are deploying personnel or responsible for the work; not the Government site where the work is being performed. If work is being performed at a contractor site, then that would also pertain to Place of Performance.
2. If the work is to be performed is a development program, include the following: Note: Work proposed to be performed under this solicitation’s effort must be associated with the physical location of the prime, principal subcontractor, critical subcontractor, and JV team members who will perform the work.
Column C Provide the CAGE Codes/DUNS numbers associated with the entity that is proposing to perform the work for this solicitation.
Note: Past performance and corporate experience of any entity with multiple CAGE codes/DUNS will be considered for the same physical location if they have distinct roles and responsibilities. If multiple CAGE codes/DUNS for the same physical location will be involved with performance or management of the requirements of this solicitation, ensure they are identified on separate lines in Table ES-1 (Offeror Summary).
| ***Note to contract specialist | Delete the reference to Corporate Experience if not using corporate experience. |
| Column D | Brief Work Description and/or Program Role and Responsibility for each CAGE Code/DUNs identified. |
Note: Provide a definition of the legal relationship between the entities if it is other than a Prime/subcontractor relationship.
| Column E | Provide the percentage of total proposed price being careful to include the prime, all subcontractors (principal subcontractors, critical subcontractors, and all other subcontractors), JV team members, or others so that the total proposed price adds up to 100% of the contract price. If there are current and previous CAGE codes/DUNS as a result of a corporate acquisition, reorganization, merger, or other administrative reason, that refer to the same division in the same physical location for where the work is being performed or from where the work is being directed place N/A in this column and ensure the proposed price/work for this effort is covered under the current CAGE codes/DUNs. |
| ***Notes to contract specialist | 1. If you have an IDIQ or services type contract, you may want to consider using the % of proposed work performed. For example an IDIQ may be priced at the individual delivery order level. |
2. If not using “all subcontractors (principal subcontractors, critical subcontractors, and all other subcontractors)”, modify the instructions and delete the last row in the table requiring that the "% of Total Proposed Cost/Price adds up to 100%".
3. For procurements where work is performed at a government site, Place of Performance pertains to the location of the company that is deploying personnel or responsible for the work; not the Government site where the work is being performed. If work is being performed at a contractor site, then that would also pertain to Place of Performance.
4. Past Performance and Corporate Experience information is only requested for the Prime and Principal and Critical Subcontractors.
| Instructions for the ES-2 Relevant Contract Summary Table | |
| Row 3 | Fill in the Offeror's Name. |
Column A List the names of the Prime, principal subcontractors, critical subcontractors, and JV team members proposing for this solicitation. Add rows as necessary.
Column B Provide a unique contract reference number for all contracts proposed as relevant for Past Performance and/or Corporate Experience. Utilize P1...Pn for the prime or Joint Venture Team proposing for this solicitation (regardless if the referenced contract was performed as a Prime or subcontractor), utilize S1...Sn for contractors proposed as subcontractors for this solicitation (regardless if the referenced contract was performed as a prime or subcontractor), and utilize T1...Tn for each JV team member proposed as part of a Joint Venture for this solicitation (only include contract references labeled as T1…Tn where the same Joint Venture Team members previously performed as a Joint Venture Team).
Note (1): Group the references by each entity where entity is defined as the Prime, principal subcontractors, critical subcontractors, or JV Team Member.
Note (2): 'n' is defined as the total number of contracts referenced for each type of entity.
Note (3): The contract references shall be numbered consecutively across all subcontractors and JV Team Members (as illustrated in the example tab) so that they are unique (e.g., there is only one P1, S1, T1).
Column C Provide the Contract Number (e.g., N00019-12-R-0001) for the referenced contract.
Column D Include the Contract Type (CPIF etc) for the referenced contract.
Column E Include the Contract Name for the referenced contract.
| Column F | Place an X in this column if the referenced contract is included in the Corporate Experience Volume. |
| ***Note to contract specialist | Delete if not using Corporate Experience |
| Column G | Place an X in this column if the referenced contract is included in the Past Performance Volume. |
Note: Ensure that contracts identified in Past Performance are numbered consistently with those identified in Corporate Experience (i.e. P1 in Past Performance is the same as P1 in Corporate Experience, etc.). The references designated in Table ES-2 Relevant Contract Summary, shall be used in completing Volume 4 Table CE-1 Evaluation Area Summary, and Volume 5 Table PP-1 Past Performance Contract Data, and should be consistent throughout.
