(4) Section 4 – Past Performance Assessment Questionnaire

For all contracts identified in Section 1, Offerors shall complete Part I of the Past Performance Assessment and email the questionnaire to both the Government contracting activity and technical representative responsible for the associated contract. The Point of Contact (POC) should electronically complete Part II of the questionnaire and email the entire questionnaire to the Contract Specialist within thirty (30) calendar days of the solicitation’s release. WOSB % %

HUBZone SB % %

SDVOSB % %

*Offerors which qualify as small businesses shall enter “N/A” in this column

L.3.2.3.1 Small Business Participation Plan (Evaluated in Section M.3.3.1, Factor 3, Subfactor 1: SB

The Offeror shall identify the extent and nature of participation of SB, WOSBs, HUBZone SBs, and SDVOSBs in the performance of the proposed contract. j. If applicable, for each category in which the Offeror’s SB Participation Plan contains goals below those described below, a detailed explanation as to why.

SB Participation Plan Objectives.

The Government’s Participation objectives for this acquisition are:

Small Business: 5.25%

Small Disadvantaged Business: 0.4%

Women-Owned Small Business: 0.4%

HUBZone Small Business: 0.4%

Service-Disabled Veteran-Owned Small Business: 0.4%

L.3.2.3.2 SB Subcontracting Plan - LARGE BUSINESSES ONLY (Evaluated in Section M.3.3.2, Factor 3, Subfactor 2: SB Subcontracting Plan)

The SB Subcontracting Plan is only required from large business Offerors (i.e., not a SB, SDB, WOSB, HUBZone

SB or SDVOSB) and will be incorporated into any resultant contract. The SB Subcontracting Plan describes the

Offeror’s approach to maximizing small businesses opportunities in this effort relative to the subcontracted portion’s value. The Offerors shall present their SB Subcontracting Plan percentages in terms of total dollars of work for each

SB category divided by the total value of subcontracts.

FAR 52.219-9 sets forth the SB Subcontracting Plan’s required elements. Offerors should address their SB

Subcontracting Plans’ elements in an order similar to the FAR order to demonstrate that they meet each element of the SB Subcontracting Plan. The Offeror shall provide the CAGE code or DUNS number for each proposed subcontractor and include the NAICS code (appropriate to the principal nature of the planned subcontract) supporting each proposed subcontractor’s size status. The information in the Offeror’s SB Subcontracting Plan must correlate with its SB Participation Plan.

Small Business: 36.7%

Small Disadvantaged Business: 5%

Women-Owned Small Business: 5%

HUBZone Small Business: 3%

Service-Disabled Veteran-Owned Small Business: 3%

Offerors proposing a SB Subcontracting Plan with percentages below the Government’s subcontracting objectives shall, for each category below the objective, explain in detail why they are unable to meet the objectives and what measures, if any, they will take to meet the objectives.

L.3.2.4 Volume IV – Business Proposal (Evaluated in Section M.3.4, Factor 4: Price)

Failing to provide a complete business submission may result in the Government disqualifying an Offeror’s proposal from competition. Offerors seeking any exception to the solicitation’s terms and conditions shall clearly explain the exception and its basis in their cover letter. Exceptions, however, may render the Offeror’s proposal unacceptable.

Provisions in the hard copy marked “original” in the Business Proposal govern in the case of any conflicts between the hard copy and the electronic version.

L.3.2.4.1 The Offer

The Business Proposal shall include a completed, signed copy of the entire solicitation with all appropriate “fill-in” sections completed as follows:

Section A: An official having the authority to bind the Offeror contractually must sign the SF33. If the individual is not an officer of the company, the proposal should include written evidence of the individual’s authority to bind the Offeror under contract to a particular course of action, such as a written delegation of authority signed by an officer of the company.

Section B: Follow the below instructions for particular Contract Line Item Number (CLIN) pricing:

o Offerors shall propose a unit price for each of the five ordering periods for CLINs 0007 &

0008.

o For CLINs 0001, 0002 & 0003 Offerors shall propose a single blended unit price for all sizes and enter the unit prices and total amounts for each CLIN.

o For CLINs 0004, 0005, 0006 and 0008 Offerors shall enter unit prices and total amounts for each CLIN. Note: For CLIN 0008, Offerors shall enter the unit price for ordering period 5 to calculate the “maximum amount.”
