About this file

This document is a Past Performance Questionnaire (Attachment S-2) for Solicitation M67004-25-R-0002, designed to collect detailed performance information about a contractor's previous work. The questionnaire requests comprehensive evaluation across six key performance areas: Quality, Schedule, Cost Control, Management, Utilization of Small Business, and Regulatory Compliance. Evaluators are asked to rate the contractor's performance on various sub-elements within each area using a five-point scale ranging from Unsatisfactory to Exceptional, with space provided for narrative explanations.

The form is intended to be completed by a previous customer or government technical representative to help the contracting agency assess a contractor's past performance for a competitive service contract. The evaluation covers critical aspects such as conformance to contract requirements, timeliness of performance, cost management, problem resolution, customer satisfaction, small business engagement, and compliance with regulations. A notable provision indicates that contractors without past performance records will receive a neutral rating, highlighting the importance of this documentation in the source selection process.
