About this file
This is an Appendix A Required Forms List for a structured cabling Request for Proposal issued by Lake Orion Community Schools in Michigan. The RFP solicits proposals for structured cabling installation and upgrades across multiple school buildings, including the Early Childhood Center, Lake Orion High School, Paint Creek Elementary, Stadium Drive Elementary, Webber Elementary, Carpenter Elementary, and Orion Oaks Elementary. The project encompasses installation of Category 6 and Category 6A cabling, patch panels, backbone fiber optics, wireless access points, power distribution units, and network closet equipment across the district. Quantities include 1,402 general data cable drops, 376 audio/visual and data cables, 445 PA/clock cables, 240 wireless access point cables, 14 network closets to be renovated, and 3 new network closets to be created. The bidder response must include completed forms in Microsoft Excel format covering proposal summary, non-familial relationship affidavit, school safety affidavit, compliance exceptions, Iran Economic Sanctions Act certification, vendor questionnaire, specifications, quantities, and cost information. Extended unit pricing will be made available to other Oakland County school districts.
All bid responses must include detailed equipment specifications with vendor compliance or exception statements, project personnel composition, service and support capabilities, and implementation schedule confirmation. Vendors must provide three project references, bonding limits, insurance documentation, and clarification of assumptions made in proposals. The RFP requires vendors to hold pricing through 2025 to accommodate phased renovations across all buildings or provide guaranteed pricing reductions on educational list prices or maximum percentage increases for cabling costs. All proposed cabling systems must include a certified 15-year performance guarantee. The bid requires submission of a performance bond with specified percentages and includes unit costs for various installation scenarios, equipment removals, and demolition work. Vendors must be certified to install the specified cabling manufacturers and possess necessary tools to certify Category 6A cabling infrastructure.
