About this file

This is an LGBTQ Applicant Opportunity Data Form used by the City of Philadelphia to collect diversity and inclusion information from vendors and contractors seeking to do business with the municipality. The form is designed to identify companies beneficially owned and controlled by Lesbian, Gay, Bisexual, Transgender, and Queer (LGBTQ) persons and to support the City's commitment to diversity, equity, and inclusion in procurement activities. Applicants are asked to self-identify their LGBTQ business status, provide current certification information from the National LGBT Chamber of Commerce (NGLCC) through its Philadelphia affiliate, the Independence Business Alliance (IBA), and identify any LGBTQ-certified subcontractors participating in the project.

The form collects voluntary data regarding additional third-party certifications held by LGBTQ-certified businesses, such as those from the Eastern Minority Supplier Development Council (ESMDC), Pennsylvania Unified Certification Program (PAUCP), Disability:IN, or Women's Business Enterprise National Council (WBENC). The City of Philadelphia has indicated that submitted data may be used at its discretion for other projects and may be made publicly available to assist in incorporating LGBTQ certified businesses into the City's vendor database. Submission of the form is entirely voluntary, and applicants must provide their business name and the associated bid or opportunity number for reference purposes.
