About this file
This is a Past Performance Questionnaire for solicitation 030ADV27R0001, a Library of Congress IDIQ contract competition for Digital Talking Book Machines (DTBM). The questionnaire is designed to collect feedback from current or previous customers regarding contractor performance and suitability to provide services to the Library. Respondents are instructed to evaluate contractors using five rating categories—Outstanding, Good, Satisfactory, Marginal, and Unsatisfactory—and must provide narrative explanations for any ratings below Good or marked as Not Applicable. Completed questionnaires must be returned to Colleen Daly (cdaly@loc.gov) and Brian Curl (bcurl@loc.gov) no later than September 14, 2026.
The questionnaire addresses ten evaluation criteria covering overall contract management, product and service quality, project phase management, schedule compliance, reporting timeliness, subcontractor performance, key personnel competency and communication, cost control, and any major contract terminations or rescheduling. Respondents are requested to provide general information about their company, point of contact, contract number, contract type, period of performance, contract dollar value, and contract description. Handwritten responses are acceptable. This questionnaire supports the evaluation of proposals for two performance areas: PA1 covering DA2 production, parts, and support, and PA2 covering existing device support and repair services for approximately 63,000 incumbent DTBM DA2 units currently in distribution.
