About this file
This is a Past Performance Questionnaire (PPQ) for solicitation 030ADV27R0001, issued by the Library of Congress for Digital Talking Book Machines. The questionnaire is designed to collect feedback from current or previous customers of contractors bidding on this acquisition, as past performance is considered of considerable importance to the evaluation process.
The questionnaire requests evaluators to assess contractor performance across ten key areas using a five-point rating scale: Outstanding, Good, Satisfactory, Marginal, and Unsatisfactory. Evaluation criteria include overall contract management, quality of products and services, project phase management, schedule compliance, timely report submission, subcontractor performance, key personnel competency and communication, cost management, and any major contract terminations or rescheduling due to performance deficiencies. Respondents must provide narrative explanations for any Marginal, Unsatisfactory, or Not Applicable ratings and are requested to return completed questionnaires via email to Colleen Daly (cdaly@loc.gov) and Brian Curl (bcurl@loc.gov) by the solicitation closing date. The questionnaire also requests general information including company name, point of contact details, contract number, contract type, period of performance, approximate dollar value, and contract description to contextualize the performance assessment.
