About this file

This is a DA Form 3479-1-R, a Trainee/Controller Evaluation form used by the United States Army Training and Doctrine Command (TRADOC) to assess air traffic control trainees and controllers. The form is referenced in FM 3-04.303 and was last updated in June 2003.

The evaluation instrument contains eight major assessment categories with specific performance expectations: Separation (ensuring proper aircraft separation), Control Judgement (awareness, planning, and positive control), Traffic Management (error correction, traffic flow, aircraft identity, professionalism, and flight strip accuracy), Operating Methods and Procedures (clearance delivery, adherence to letters of agreement, navigational and weather assistance, and handoff procedures), Coordination and Communication (traffic advisories, coordination thoroughness, and transmission accuracy), Phraseology (standard phraseology adherence, voice quality, and speech rate), Equipment (understanding and utilization of equipment capabilities), and Other (customizable assessment areas). Each evaluation factor is rated as Satisfactory (SAT), Needs Improvement (NEEDS IMPROV), or Unsatisfactory (UNSAT). The form also captures traffic condition information (Light, Moderate, or Heavy; Stable or Fluctuating) and provides spaces for evaluator comments, overall rating, trainee/controller response (agree or disagree with evaluation), and reviewing authority comments. All evaluation activities require dated signatures from the evaluator, trainee/controller, and reviewing authority.
