About this file
This is an Indian Economic Enterprise (IEE) Representation Form from the Indian Health Service (IHS), Department of Health & Human Services, for solicitation #75H70125R00037 titled "TPPIHC Roof Replacement and Fall Protection." The form is used to self-certify that an offeror meets the definition of an "Indian Economic Enterprise" under the Buy Indian Act (25 U.S.C. 47).
The form requires offerors to maintain IEE eligibility throughout three key periods: when making the offer, at contract award, and during the full contract performance period. Offerors must be registered in the System for Award Management (SAM) and may need to provide documentation of eligibility. The form includes spaces for the offeror to indicate whether they meet the IEE definition, and requires details such as the name of the 51% owner, federally recognized tribal entity, business name, DUNS number, and certifying signature. False representations are subject to penalties under 18 U.S.C. 1001 and other applicable laws.
