About this file

This is a Responsibility Inquiry form (Attachment F) issued by the Department of Administrative Services (DAS) Procurement Services for ITB S-63500-00015960 regarding the Applegate Gravel Augmentation project in Oregon. The project involves placing 500 cubic yards of gravel in the Applegate River approximately 0.6 miles downstream of the Applegate Dam. The form was issued on February 20, 2019 and serves as a pre-award evaluation document to assess bidder capability and responsibility prior to contract execution.

The Responsibility Inquiry form requires bidders to provide detailed responses addressing eight key evaluation areas: financial, material, equipment, facility, and personnel resource availability; contract performance history over the past five years regarding schedule and budget adherence; criminal or civil charges within the past three years for fraud-related offenses or antitrust violations; contract termination history and litigation within the past three years; outstanding or pending judgments and financial distress indicators; bankruptcy or insolvency actions within the past three years; current licensing, insurance, and authorization to do business in Oregon; and possession of an unexpired Pay Equity Certificate issued by DAS. The form requires an authorized representative signature certifying that all responses are complete, accurate, and not misleading. The agency reserves the right to request additional documentation including profit-and-loss statements, balance sheets, cash flow information, bonding capacity, credit information, and performance records, with failure to provide such information potentially resulting in a non-responsibility determination and bid rejection.
