Summary: Exhibit D – Application for Payment and Invoice Requirements for CDBG Infrastructure Improvement Project, City of Milton, Florida
Exhibit D establishes the application for payment and invoice requirements for the City of Milton's CDBG Infrastructure Improvement Project (Contract No. CDBG-CON-01). The contractor must submit one application for payment per month covering work performed through a consistent monthly cutoff date. This is a unit-price contract where compensation for unit-price line items is based on actual quantities of work performed, measured and verified by the City or Engineer of Record, multiplied by established unit prices. Lump-sum line items are paid based on percentage completion as approved by the Engineer. All applications must include AIA Form G702 (Application and Certification for Payment) signed by the contractor and certified by the Engineer, AIA Form G703 (Continuation Sheet) itemizing each schedule of values line item with quantities and completion percentages, the contractor's certification that work is complete per contract documents, and a statement summarizing contract sum, net changes, retainage, earned amounts, and current payment due.
Supporting documentation required with each application includes certified weekly payrolls on DOL Form WH-347 covering all payroll periods, Buy America/BABA mill certificates and manufacturer certifications for applicable materials, Section 3 labor-hour reporting data, M/WBE subcontract and purchase award documentation, stored materials documentation with supplier invoices and proof of proper storage and insurance, and Change Order substantiation for any approved changes. The City will withhold retainage not to exceed five percent of each progress payment in accordance with Florida Statute 255.078, and will cease withholding additional retainage once work is fifty percent complete if progress is satisfactory. Applications must be submitted by email in a consolidated PDF to three designated City representatives: Sandra Woodbery, April Taylor, and Joe Cook. The City's agent (Engineer of Record) must review and certify each application, with rejection possible within twenty business days if deficient. Payment of proper, approved applications is due twenty-five business days after recorded receipt. The contractor must remit payment to subcontractors and suppliers within ten days of receiving City payment. Final applications must include all closeout submittals, final certified payrolls, final lien waivers, subcontractor payment affidavits, surety consent, and the Engineer's Project Completion Certificate.
