About this file

This is a Disadvantaged Business Enterprise (DBE) and Non-DBE Subcontractor Utilization Form for the City of Ocala, Florida's construction project to build and install fifteen bus stop shelters throughout Ocala and Marion County. The form serves as a certification document that prime contractors must complete and submit with their bid packages to document their use of certified DBE subcontractors and the estimated dollar amounts for each subcontract. Contractors are required to make assertive good faith efforts to utilize as many DBEs as possible on this project and must maintain a copy of the completed form on file. The form requires bidders to either commit to no DBE subcontractors if performing all work in-house, commit to a minimum of 2.55% DBE utilization, or submit documentation of good faith efforts if unable to meet the 2.55% goal.

The form mandates that all participating subcontractors—both DBE and non-DBE—be listed with their names, addresses, DBE certification status, descriptions of work to be performed, and the agreed-upon contract amounts. DBE status must be verified through the Unified Certification Program; firms without certification are classified as non-DBE. Bidders must certify that the information provided is true and complete and acknowledge that failure to provide accurate information or exercise positive good faith efforts may result in being considered non-responsive to the solicitation requirements. The City reserves the right to recommend an audit to verify submitted information, and if awarded the contract, this utilization form becomes an integral part of the contract agreement.
