About this file
This document is an Invitation to Bid (ITB) No. S-10700-00012203 from the Oregon Department of Administrative Services (DAS) Procurement Services for the Supply of Bulk Propane Fuel, Tanks, Related Goods and Services. The document is Attachment B, a Bidder Information and Certification Form, which requires potential vendors to complete and submit detailed certifications regarding their business practices, compliance, and eligibility to bid on the contract. The bid is valid for 180 days following the closing date, and successful bidders will be expected to enter into a Price Agreement with performance standards.
The certification form requires bidders to affirm multiple compliance requirements, including non-discrimination in employment practices, tax law compliance, absence of conflicts of interest, and adherence to various Oregon Revised Statutes (ORS). Specifically, bidders must certify they do not discriminate against employees or subcontractors, have policies preventing sexual harassment, comply with wage and compensation laws, and are not included on federal exclusion lists. The document also emphasizes that any false statements or misrepresentations could result in penalties under the Oregon False Claims Act, and continuing compliance is a material element of the potential Price Agreement.
