About this file

This is an Invitation for Bid document issued by the State of Illinois for vendors to submit offers. The document serves as the formal bidding form that requires vendors to complete and submit all required documentation as referenced in solicitation section A.1. Bidders must ensure full compliance with all solicitation requirements, as failure to meet specifications will result in disqualification. The form includes sections for bidders to declare any claims for state preferences, exceptions to standard contract terms and conditions, and authorization signatures from the bidder's representative.

The State of Illinois offers various procurement preferences under the Illinois Procurement Code to promote business opportunities within the state. Bidders claiming the Illinois Business Preference must complete and submit the provided affidavit, while those claiming the Domestic Product Preference must designate "Domestic Product" in the Alternate Description field for each applicable line item. Any bidder taking exception to the Standard Terms and Conditions must submit a redline version of those terms with their offer, though the State reserves the right to accept or reject any proposed exceptions. The Standard Terms and Conditions are available on the CPO General website at https://cpo-general.illinois.gov/solicitation-and-contract-templates.html. By signing this form, the bidder makes a binding offer to the State of Illinois for performance in full compliance with the solicitation requirements.
