About this file
This is an Insurance Schedule document for the Nevada Department of Corrections (44DOC-S3191) outlining comprehensive insurance requirements for contractors and subcontractors performing work under a state contract. The document details mandatory minimum insurance coverage specifications, including Commercial General Liability (with $2,000,000 general aggregate limit), Automobile Liability ($1,000,000 combined single limit), Workers' Compensation and Employers' Liability (with statutory coverage), and Pollution Legal Liability (with $1,000,000 per occurrence and $2,000,000 annual aggregate) if the contract involves hazardous materials.
The insurance requirements mandate that the State of Nevada must be named as an additional insured on all policies, with contractors' insurance coverage being primary and non-contributory. Insurers must be licensed in Nevada with an A.M. Best rating of not less than A-VII. Contractors must provide certificates of insurance before work commences and maintain coverage for the entire contract duration, with a 30-day notice required for policy cancellation. Failure to maintain required insurance policies is considered a material breach of contract. The document also specifies that subcontractors must be included as additional insureds, and any modifications to insurance requirements must receive prior approval from the Nevada Attorney General's Office or Risk Manager.
