Addendum to 52.212-1

I. GENERAL INSTRUCTIONS:

A. These instructions provide general guidance for preparing proposals, as well as specific instructions on the format and content of the proposal. Throughout this section, the term

"offeror" includes joint venture partners. This is a continuation of the solicitation provision FAR

52.212-1.

B. Current Federal and Defense Regulations require the use of the System for Award

Management (SAM) and Wide-Area Work Flow (WAWF) Invoicing System. A vendor must be registered in these systems prior to award of the contract otherwise the offeror may be considered nonresponsive if the attainment of these registrations delays award of the contract.

C. For consideration of award, offerors shall submit one (1) complete copy of the proposal package with all required clause fill-ins and pricing information by the closing date and time of the solicitation.

D. Send only unclassified proposals electronically.

E. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale within seven (7) working days after initial solicitation posting on beta.SAM.gov.

F. The proposal response shall consist of a copy of two (2) separate parts; Part I - Price Proposal and Part II - Past Performance Proposal.

(2) Company name

(3) Company POC & phone number

H. Submission. Submit one (1) copy of your proposal in accordance with this Proposal

J. Bidders Library. All solicitation documentation, including the Performance Work Statement

(PWS) and attachments, are available on the betaSAM website: www.beta.sam.gov. Any other documents related to the solicitation not posted to the betaSAM website will be made available on an "as needed" basis upon request to the Contracting Officer. All the documents requested will be posted on betaSAM or a notification will be posted allowing all offerors access to the same information.

K. Preference. Preference will be given to the Randolph-Sheppard Act (RSA) specified-agency for the State of Colorado (Blind Enterprise Program – BEP). Under the RSA, contracts are awarded to the RSA offeror who meets the competitive criteria, is responsive to the requirements of the contract, and has a reasonable price. While the Act does not define what a “reasonable” price is, in practice, if the BEP bid falls within the competitive range as determined by the

Contracting Officer, then the contract will be awarded to the BEP, even if the price is higher than that of the best value offer.

L. Debriefings. The Contracting Officer will promptly notify offerors of any decision to exclude them from the competitive range (see paragraph K); whereupon, they may request and receive a debriefing in accordance with FAR 15.505. Offerors excluded from the competitive range may request a pre-award debriefing or they may choose to wait until after the source selection decision to request a post-award debriefing. However, offerors excluded from the competitive range are entitled to no more than one debriefing for each proposal. The CO will notify unsuccessful offerors in the competitive range of the source selection decision in accordance with FAR 15.506. Upon such notification, unsuccessful offerors may request and receive a debriefing. Offerors desiring a debriefing must make their request in accordance with the requirements of FAR 15.505 or 15.506, as applicable.

M. Supplies. The contractor shall make every effort possible to utilize AbilityOne for all supplies that are IAW, Attachment 3 – Products Contractor Buys, of this solicitation. Additionally, the contractor shall be in compliance to the maximum extent practicable with FAR 8.005 and clause

The following additional clauses will apply: ADDENDUM 52.212-2

Addendum to 52.212-2

I. SOURCE SELECTION:

A. Basis for Contract Award

(1) This is a Price-Performance Trade-off (PPT) source selection conducted in accordance with Federal Acquisition Regulation (FAR) Part 15, Contracting by Negotiation, as supplemented by the Defense Federal Acquisition Regulation Supplement (DFARS), the Air Force Federal Acquisition Regulation Supplement (AFFARS), DoD Source

Selection Procedures, Appendix A, dated 31 March 2016 and Air Force Mandatory

Procedures 5315.3 2019 Edition. These regulations are available electronically at the

FARSite, http://www.acquisition.gov.

(2) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. However, see paragraph K in the General Instructions for preference rules.

(3) Solicitation Requirements, Terms and Conditions. Offerors are required to meet all solicitation requirements, such as terms and conditions, representations and certifications, evaluation factor requirements, and be responsible in accordance with

FAR Part 9. Failure to comply with the terms and conditions of the solicitation may result in the offeror being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and must provide complete supporting rationale. The Government reserves the right to determine any such exceptions unacceptable.

B. Evaluation Methodology

(1) The Government will evaluate proposals using two Factors: (1) Past Performance, and

(2) Price, whereas Past Performance is significantly more important than price.

(2) The Government reserves the right to award without discussions, formal discussions, to establish a competitive range IAW FAR 15.306(c). If discussions are necessary they will be conducted IAW FAR 52.212-1(g).

C. Evaluation Factors

(1) Each proposal evaluation includes:

(a) Factor 1: Past Performance

(b) Factor 2: Price

D. Factor 1 – Past Performance

(1) The Government will conduct a past performance evaluation that examines an offeror's recent and relevant past performance record to assess the Government’s confidence in the offeror’s ability to perform as proposed. There are three aspects to the past performance evaluation: recency, relevancy and quality.

(2) In order to meet the criteria for recency, the referenced contract(s) must have been performed within the past five (5) years from the date of issuance of this solicitation

(calendar years 2015-2020) for a minimum of a six (6) month period.

(3) In order to meet the criteria for relevancy, reference table below.

Levels of Relevancy

Past Performance Relevancy Ratings http://www.acquisition.gov/

Rating Definition

Very Relevant

(25,000 customers and above – monthly)

Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation requires.

Relevant

(21,000-24,999 customers – monthly)

Present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires.

Somewhat Relevant

(17,000-20,999 customers – monthly)

Present/past performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires.

Not Relevant

(less than 17,000 customers – monthly)

Present/past performance effort involved little or none of the scope and magnitude of effort and complexities this solicitation requires.

With respect to relevancy, past performance of greater relevancy will be considered a stronger predictor of future success and have more influence on the past performance confidence assessment than past performance of lesser relevance.

(4) The quality aspect of the past performance evaluation indicates how well the contractor performed on three (3) previous contracts.

(5) The past performance evaluation performed in support of a current source selection does not establish, create, or change the existing record and history of the offeror’s past performance on past contracts; rather, the past performance evaluation process gathers information from customers on how well the offeror performed those past contracts.

(6) Past performance, information obtained through the Past Performance Information

Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information

System (FAPIIS), Electronic Subcontract Reporting System (eSRS), and other databases, interviews with program managers and Contracting Officers, or other sources known to the Government.

(7) The Government will review this past performance information and determine the quality and usefulness as it applies to the performance confidence assessment. The criteria will be evaluated using the Attachment 5 (Evaluation Criteria) template. The contractor shall identify each category using Attachment 4, one (1) page per category for a total not to exceed fifteen (15) pages. The past performance confidence assessment rating is based on the criteria identified in Attachment 5 (Evaluation

Criteria). The rating scale will be as follows:

E. Factor 2 – Price

(1) The Government will evaluate offers for award purposes by adding the Total

Evaluated Price (TEP) for the Phase-In, base, all options and six-month extension. The

Government may determine that an offer is unacceptable if the option prices are significantly unbalanced.

(2) To determine overall reasonableness the price proposal will demonstrate understanding of contract requirements and ability to provide contract services. To determine a fair and reasonable price adequate price competition is expected, but the

Government may use any acceptable method as described in FAR Part 15.
