Block 4: The DD Form 1664 Data Item Description (DID) has been tailored as follows:
Para 10.2.1 Change to read: The plan shall provide a comprehensive management plan describing the transition of facilities, equipment, documentation, personnel, and all other materials and responsibilities identified in the PWS. The plan will clearly identify the phase-out between the incumbent contractor and the new contractor.
Para 10.2.2, and 10.2.4 – Delete.
Para 10.2.3 – Insert period after the word “Plan”, delete remainder of sentence.
Para 10.3.3 – Delete first sentence.
Block 7: Letter of transmittal only via e-mail to 50 CONS/PKB.
Block 12: Submit within 45 calendar days of request by CO. The contractor shall have 10 calendar days to incorporate the government comments. Once accepted by the 50FSS/FSVF distribute within 5 calendar days.
Block 13: Submit updated plan when changes occur. The government has 10 calendar days to review the preliminary document. The contractor shall have 10 calendar days to incorporate the government comments. Once accepted by the 50FSS/FSVF, distribute within 1 work day.
