About this file
The document is an Independent Contractors Minimum Requirements file for the City of Milpitas' Recreation and Community Services Department, outlining qualifications for vendors seeking to provide recreational classes. The City is requesting responses to a Request for Qualifications (RFQual) from vendors capable of offering classes in arts and music, enrichment, parent and me, sports and fitness, and technology for participants ranging from pre-k to seniors. Evaluation dates for new vendor submissions are set for September 18, December 4, and April 24, with vendors required to develop comprehensive curricula, provide professional instructors, and supply class materials. The RFQual process will pre-qualify vendors who will be contacted prior to each season to submit specific class proposals.
Vendors must meet specific requirements, including a minimum of two years of teaching or coaching experience, or provide alternative credentials such as diplomas or certificates. Insurance requirements include Commercial General Liability Insurance with a $1,000,000 policy level, workers compensation insurance (if applicable), and a valid Milpitas Business License. Vendors must also submit professional references and a W-9 form. While being pre-qualified does not guarantee class selection, the City reserves the right to approve or reject any class proposal at its discretion. Questions regarding the RFQual must be submitted through a designated Q&A portal, with responses posted as addenda to the original document.
