About this file

This is an Employer Certification form for the Wage Theft Ordinance Responsibility Criteria required by the City of Austin for contractors seeking contract awards. The document pertains to a Traffic Flares contract opportunity with the City of Austin, specifically for distribution and sale of traffic safety flares to be used by the Austin Police Department. All employers submitting offers must complete the certification prior to contract award and are required to complete City of Austin Wage Theft Training within 30 days of being awarded a contract.

The certification requires that firms attest, under penalty of perjury under Texas law, that they have not been adjudicated for wage theft-related incidents as defined in Austin City Code Chapter 4, Section 4-22-1(G) and (H) within five years prior to the solicitation posting date or agreement start date. Firms must also agree to abide by the requirements outlined in Austin City Code Section 4-22-5(B) and Section 4-22-6(A). The form requires completion with the firm's full legal entity name, signature of an authorized person, their printed name and title, location where signed, and date of certification. Questions regarding the wage theft program should be directed to wagetheft@austintexas.gov.
