PAST/PRESENT PERFORMANCE QUESTIONNAIRE
General Service Administration is conducting an acquisition in accordance with FAR Part 15. Your assistance is requested to provide the following information in an effort to evaluate the offeror’s present and past performance. Forward this completed questionnaire via e-mail Demetrius Smith, Contracting Officer at demetrius.smith@gsa.gov. If you experience any problems, please contact me at (937)431-3625. | For the Past Performance/Past Experience factor, the offeror demonstrates highly relevant experience, has consistently received superior ratings, and the Government has not identified any significant negative information. For the Technical Approach and Management and Staffing factors, the proposal clearly meets and consistently exceeds the Government’s requirements in all areas, while presenting very low risks. |

| For the Past Performance/Past Experience factor, the offeror demonstrates highly relevant experience, has received many superior ratings, and the Government has identified little negative information. For the Technical Approach and Management and Staffing factors, the proposal meets the Government’s requirements in all areas and exceeds requirements in many areas, while presenting low risks. |
| Satisfactory – (3) |
| For the Past Performance/Past Experience factor, the offeror has received some superior ratings, and the Government has identified little negative information. For the Technical Approach and Management and Staffing factors, the proposal meets the Government’s requirements in all areas and exceeds requirements in some areas, while presenting only moderate risks. |
| Marginal – (2) |
| For the Past Performance/Past Experience factor, the offeror has received few superior ratings, or the Government has identified some negative information. For the Technical Approach and Management and Staffing factors, the proposal barely meets and rarely exceeds the Government’s requirements, and presents high but not unacceptable risks. |
| Unsatisfactory – (1) |
| For the Past Performance/Past Experience factor, the offeror has delivered unacceptable performance in a significant number of instances, or has a significant number of not acceptable ratings. For the Technical Approach and Management and Staffing factors, the proposal fails to meet the Government’s requirements, and/or presents unacceptable risks. Offerors receiving Not Acceptable ratings for any factor will not be eligible for award. |
Ratings of higher or lower than satisfactory should include a narrative justification.
