About this file

This is the Indian Affairs Indian Economic Enterprise Representation Form, a self-certification document required for offerors responding to Department of the Interior solicitations set aside under the Buy Indian Act (25 U.S.C. 47). The form establishes that an enterprise meets the definition of an "Indian Economic Enterprise" (IEE) as defined in the Department of the Interior Acquisition Regulation (DIAR) Part 1480. Offerors must complete this form at the time of offer submission, at contract award, and maintain compliance throughout the full contract term. The Contracting Officer may request additional eligibility documentation at any point during the acquisition lifecycle.

The form requires offerors to provide their federally recognized tribal entity name, Unique Entity ID (UEI), legal business name, the name of the 51% or greater owner of the IEE firm, and a certifying signature with printed name. False or misleading information submitted in connection with a Buy Indian Act award set-aside violates 18 U.S.C. 1001, while false claims during contract performance are subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287. The form also includes header fields for the solicitation/sources sought/RFI number, project title, and date of form submission.
