The Prescriptions are processed at the WRNMMC in MD using the NPI assigned to the WALTER REED SOLDIER'S HOME PHCY and transported to WALTER REED SOLDIER'S HOME PHCY by van. No active Pharmacy at the actual WALTER REED SOLDIER'S HOME. The LBN is correct Walter Reed National Military Medical Center. We are waiting for them to update their TAX ID Letter which still shows the LBN as National Navy Medical Center. If Rosemary changes LBN at NPPES prior to the TAX ID change, it might cause some confusion.

BARBERS PT PHCY 1204104

There is a physician entering prescriptions at Barbers Point. The scripts are processed through AHLTA with the Barbers Point NPI and sent elsewhere in the Honolulu vicinity for processing . Rosemary says there are no reversals. This has been referred to DHA in Virginia.

4. Per Preston email 12/22/14:

The attachment, "PharmacyClosing.xlsx", contains a listing of pharmacies that we have a completed Pharmacy Termination Notification form on. This is the form that Rose spoke about during our meeting on Friday. I opened each form (241 files) and copied the data into the attached file. I did a spot check to see if any of the listed pharmacies are in the "PASS_MTF_CCL_12182014" file that I sent last week; I found that they were listed in the "Raw" (Active pharmacy listing) and "Inactive" spreadsheet.

Sandy used the tab "168" from Preston's spreadsheet "PharmacyClosing.xlsx" to map Closing Effective date to column labelled "NCPDP Closed Date" , set status to INACTIVE, and added comment "Previously closed".

Per 12/23/14 email from HM. Remove the dates ESI entered. They were place holders. Active sites only get activation dates.

Expiration date will only be used for closed pharmacies.

SR. Sites with no transactions have no dates.

Per Preston's 1/6 email (Changes applied to 12/23/14 spreadsheet):

1) The file contains spreadsheet "168" which contains a listing of 175 terminated pharmacies and their NCPDP numbers; there are 4 duplicate NCPDP numbers.

2) The file does not contain "Phcy w-shared NPIs-Inactive" spreadsheet that was in the PharmacyClosing file that I previously sent out. That spreadsheet contained a listing of 66 terminated pharmacies and their NCPDP numbers.

3) There are 234 unique NCPDP number listed from the "168" and "Phcy w-shared NPIs-Inactive" spreadsheets.

4) ) There are 6 pharmacies in which we have two termination forms on file. I entered a comment for each of these pharmacies in column U on the "Working" spreadsheet.

Actions / Discussion per 1/5/14 Meeting with H. Morales.

1. Look in Column V STATUS at sites that have not yet been clearly identified as ACTIVE or INACTIVE and filter out sites that with a transaction sometime in 2014, there are still 225 sites, some whose last transaction was years ago and 40 whose Facility Type Code (from the DMIS Table) is INACTIVE. What do we do with these?

1/5/15: Temporarily tag sites one of three ways: ACTIVE, INACTIVE (LAST ACTIVITY WITHIN 5/1/13-5/1/15), INACTIVE -Long Term (LT) (Closed and no activity since 5/1/13.)

Ultimately, the main delivery tab will only include ACTIVE and INACTIVE (Short Term).

2. Confirm from Hector's emails whether or not zip code remains.

Per 11/23/14 email from Hector, ZIP remains for Sheyla Keane, LM, supporting Analytics Branch.

3. Confer with Hector to determine dates to put in column "exp date".

ACTIVE sites will have no dates.

We don't seem to have dates for all INACTIVE sites. What to do?

Also, there is some discrepancy between "Last Rx Dispensed Date" and "NCPDP Closed Date". Preference?

Resolution:

1/5/15:

ACTIVE sites will have no dates.

INACTIVE-LT sites will not be delivered. Dates a relative non-issure, except that ultimately, we'll keep Closed Date.

INACTIVE - Keep last transaction date.

4. Confirm with Hector whether INACTIVE sites are to be removed for both EMDEON/ESI and for Keane (Analytics Branch).

1/5/15:

5. Revert to Service/Fac pre-Joelle change but let Hector know if there will be new values.

SR reviewed data and found that SVC and related SVC CMD were different in the two original spreadsheets. (Original PASS chain code list (Preston's list) and the Analytics Branch list (Keane) . I took the following steps:

a. Reinstated Preston's values for Service.

b. Compared Preston's values to Keane's values and identiified discrepant sites. Choose a final value as follows:

If the site is inactive (pharmacy labelled inactive, DMIS labelled inactive, or no transactions 2014 forward) , retain Preston's values.
If Preston's value matches the SVC for the DMIS as reported on the DMIS list, retain Preston's values.
If Preston said JOINT and the site was at Bethesda, Pentagon, Belvoir, Woodbridge, or DiLorenzo, use JOINT.
If Preston's value was corrected by Jo-El Onstad (UBO), use Jo-El's value.

INACTIVE -LT - Last transaction prior to 5/1/13, OR no transactions ever and NCPDP deactivated OR ever or no transactions since 5/1/13 to INACTIVE -ST - Last transaction in the 5/1/13 - 5/1/15 period but NCPDP closed.

blank - No reason to believe site is inactive.

5. Preston inserted column s "V". Data was copied from the Pharmacy Termination Notification forms to column U and V. There are 6 pharmacies that have two termination notifications forms. A comment was entered in column U for each of these pharmacies.

6. With decisions above, create final WORKING .xlsx.

7. With final WORKING.xlsx, create two deliverable versions.

The first three rows on the tab Working List are indications of the variable names and columns that will remain when we generate our final two deliverables:

PASS_MTF_CCL_mmddyyyy for Emdeon/ESI, per Row 2, Yellow Highlighted columns MTF_Chain_Code_List_mmddyyyy for Sheyla Keane, LM, supporting Analytics Branch, per Row 1, Blue Highlighted Columns.

In final review of deliverables, ensure the following:

All VLOOKUP , calculations, or in-cell comments are removed .

All formatting within rows, e.g., color coding, bolds, etc. is removed INACTIVE sites are moved off the main tab according to result of #4 above.

Three output files:

CHC receives Active and Inactive tab. Columns with yellow headers. Any code used in the W/H should not be reused. There may be no duplicates.

DSC: Active and Inactive, columns with blue header.

ESI: Active tab only. Columns with green header.

MCCLELLAN PHCY:

Roger identified RXs that were processed in Retail and VA CHDR that had a corresponding Site MTF name of McClellan associated with the RXs.

Only MTF transactions should have a Site MTF name associated with their RXs. All other Service Categories, Retail, Mail Order, Theater and VA CHDR, RXs should show a blank space when a Site MTF column is included in the output along with the Service Category (Claim). When I ran a follow up report to find out more details of the RXs with a Site MTF name of McClellan in this same time period I found that the Site MTF name and even a Branch of Service of Air Force was being linked to these transactions. The pharmacy processing these RXs is the McClellan VA CBOC Pharmacy with an NPI number of 1588612329.

Emdeon uses both the Active and Inactive tabs to generate its master pharmacy list. Leaving this site on the Inactive tab causes the situation above. Only by removing the site from the list altogether can the Emdeon problem be solved.

This does not apply to NPI 1396842159, named MCCLELLAN DGMC VA PHARMCY, also showing a Air Force Branch of Service and the same chain code of 0BG. This pharmacy, when looked up on SelectRX shows the same pharmacy name with an address of 5342 Dudley BLVD, McClellan, CA 95652 and a chain code of 0BG.
